| Objective:Combined with our experience to investigate the clinical features,diagnosis and prognosis of chromophobe renal cell carcinoma(CRCC).Methods:from January 1,2011 to December 1,2018,44 cases of chromophobe renal cell carcinoma were treated in our hospital,all of them were accepted retroperitoneal laparoscopy.We collected and retrospectively analyzed the clinical symptoms,imaging and pathological data,surgical procedures and prognosis.Results:The incidence rate of CRCC was 2.2%0(44/1987)in renal cell carcinoma.the median age of 44 patients was 54.5 years(26-72 years),there were 23 males(52%)and 21 females(48%).and M/F ratio was 1.09:1.and 31 cases(70%)were found by chance,13 cases(30%)have clinical symptoms,The main symptom is lumbago。The median size of the tumor was 4.3cm(1.6-13cm),There were 24 cases(55%)on the left side and 19 cases(44%)on the right side.One patient had bilateral tumor and one patient had multiple lesions on one side.Laparoscopic radical nephrectomy(RN)was performed in 21 cases(48%)and laparoscopic Nephron-sparing surgery(NSS)in 23 cases(52%).All the surgical margin was negative after partial nephrectomy,and no tumor was found at the end of ureter and blood vessel and one case invaded the mucosa of renal pelvis after radical resection。No enlargement of renal hilar lymph nodes was found during the operation,and no sarcomatoid changes were found in postoperative pathology.There were 33 cases(75%)in pT1 stage and 5 cases in pT2 stage,no metastasis was found.The median follow-up was 36 months(1-84 months).No dead patients were found and no patient have recurrence.The operative time,blood loss and complications of Nephron-sparing surgery and radical nephrectomy were not statistically significant(p>0.05).The 1-,2-and 3-year survival rates were the same between the two groups.Conclusion:renal chromophobe cell carcinoma(CRCC)is a rare type of renal cell carcinoma with low malignancy and its clinical symptoms are not obvious.The diagnosis needs to be based on typical pathological features.n combination with our experience,renal chromophobe cells are less likely to metastasize and recrudesce and have a good prognosis,possibly due to the high incidence of low staging.There was no significant difference between laparoscopic Nephron-sparing surgery and laparoscopic radical nephrectomy.The results of oncology in the two groups were similar.Nephron-sparing surgery should be recommended clinically. |
