Objective: Appplication of optical coherence tomography angiography(OCTA)to scan macular and peripapillary region with different degrees of eyes to compare the variation of 3 mm× 3 mm macular vessel density and retinal thickness,also to compare the variation of 4.5mm× 4.5 mm peripapillary vessel density and the thickness of retinal nerve fiber layer.To discuss the effect of axial length,refractive error on vessel density and retinal thickness with different degrees of myopia.Method: Controlled clinical studies.A total of 198 eyes of 100 subjects with different degrees of eyes were included,which was divided into 3 groups on the basis of the refractive status: 31 eyes as control group,55 eyes with moderate myopia,and 62 eyes with high myopia.FAZ、3 mm× 3 mm macular vessel density and retinal thickness 、4.5mm× 4.5 mm peripapillary vessel density and the thickness of retinal nerve fiber layer were determined by OCTA.Using Lenstar LS900 to check the axial length.Correlations of the main outcomes were analyzed.Using SPSS 23.0 statistical software to process data.Result: FAZ in the control group(0.331±0.115)mm2 was 0.096mm2 larger than that in the high myopia group(0.235±0.105)mm2,and the difference was statistically significant(P<0.001).The difference of superficial foveal vessel density between control group and high myopia group was statistically significant(P <0.001),moderate myopia group and high myopia group(P =0.008).The difference of deep foveal vessel density between control group and moderate myopia group was statistically significant(P =0.012),control group and high myopia group(P<0.001).There was no statistically significant difference among the three groups in superficial parafoveal vessel density(P > 0.05).The deep parafoveal vessel density of the control group was significantly lower than that of the moderate myopia group and the high myopia group(P<0.001,P<0.05).Superior(S),inferior(I),nasal(N),and temporal(T)of the superficial parafoveal vessel density was no significant difference among three groups(P > 0.05).The difference of each quadrant of deep parafoveal vessel density was statistically significant(P<0.05).There were statistically significant differences in the whole image,parafoveal and quadrant thickness between the control group and the moderate myopia group(P<0.05).the control group and the high myopia group(P<0.001).The peripapillary vessel density of the whole image,the vessel density within the optic disc and the vessel density besides the optic disc were statistically significant between the control group and the high myopia group(P =0.025,P <0.001,P =0.001).The NS、NI quadrant vessel density of the control group and the moderate myopia group showed statistically significant differences from the high myopia group.There was a statistically significant difference between the moderate myopia group and the high myopia group in the nasal quadrant vessel density(P =0.007).The difference between the control group and the high myopia group was statistically significant in the inferior quadrant vessel density(P =0.044).There was a significant positive correlation between the superficial and deep foveal vessel density with axial length.The thickness of macular area was significantly correlated with axial length and refractive state.There was a significant positive correlation between the vessel density within the optic disc and the axial length(r=0.182,P=0.003).The nasal quadrant vessel density was negatively correlated with the axial length(r=-0.169,P=0.04).In addition to the superior quadrant,the RNFL thickness of nasal、inferior and the temporal quadrant were significantly correlated with the axial length(r=-0.215,P=0.009;r=-0.289,P<0.001;r=0.235,P=0.004).Conclusion:1.With the increase of myopia degree,the axial length increased,the FAZ was smaller,while the superficial and deep vessel density of foveal increased,which was positively correlated with the axial length;There was no difference in the superficial parafoveal vessel density between the three groups,while there was a difference in the deep parafoveal vessel density between the three groups.The deep parafoveal vessel density of control group-moderate myopia-high myopia showed a downward trend after an increase.The thickness of foveal was thicker when degree deepening and axial growth,while the parafoveal was thinner.2.The peripapillary vessel density of the whole image,the vessel density within the optic disc and the vessel density besides the optic disc were statistically significant between the control group and the high myopia group.The RNFL thickness in the nasal、inferior was negative correlated with the axial length,in which the temporal RNFL thickness was positive correlated with the axial growth.
