| PURPOSE: The purpose of this study was to investigate the effect of preoperative oral carbohydrate on preoperative and postoperative blood glucose levels and insulin resistance in patients with gastrointestinal tumors,and to provide a new method for clinical fasting and abstinence before and after surgery.METHODS: In this paper,97 patients who underwent gastrointestinal surgery in the department of gastroenterology of the first affiliated hospital of fujian medical university from July 2018 to March 2019 were collected and randomly divided into the experimental group(48 cases)and the control group(49 cases)according to the order of even Numbers.In the experimental group,800 ml of carbohydrates were taken orally 12 hous before the surgery,and 400 ml of carbohydrates were taken orally 4 hours before the surgery.Here,we applied a special formula for medical use: tuyi carbohydrate component.In the control group,preoperative routine abstinence from food and drink and no oral carbohydrate were observed.The blood glucose(FPG),insulin level(FINS),and insulin resistance index homeostatic model(HOMA-IR)of the two groups of patients were measured at admission,in the morning after surgery,and immediately after surgery.Visual analogue scale scoring was used to evaluate the subjective comfort of the patients 30 min before surgery,including 6 components including thirst,hunger,nausea,anxiety,fatigue,and weakness.RESULTS: 1.Baseline data: age,gender,body mass index,previous abdominal surgery history,admitted blood glucose,insulin level and insulin resistance index of patients in the two groups were compared,the difference was not statistically significant(P > 0.05),and the two groups were comparable.2.Subjective comfort: compared with the control group,the levels of thirst,hunger and anxiety in the experimental group were significantly reduced,with statistically significant differences(P < 0.05),while the levels of nausea,fatigue and weakness were not statistically significant(P > 0.05).3.Blood glucose level: there were no cases in the experimental group with lower than normal morning blood glucose,and 5 cases in the control group(10.2%)with lower than normal morning blood glucose,and the incidence of lower blood glucose was higher than that in the experimental group.The difference of morning blood glucose level between the two groups was statistically significant(P < 0.05).Experimental group and control group in postoperative blood glucose were higher than normal ceiling,10 cases in the control group had significantly higher(> 10.0 tendency/l),but only 4 cases of experimental group increased significantly,the difference was statistically significant(P < 0.05)and postoperative insulin resistance index: two groups of patients with postoperative insulin levels,insulin resistance index is higher than preoperative,insulin resistance index increased more obviously,but the control group was statistically significant difference(P < 0.05).CONCLUSION: carbohydrate can improve the subjective comfort of patients,and significantly alleviate the preoperative hunger,thirst,anxiety and other uncomfortable reactions of patients.Can make preoperative and postoperative blood glucose level more stable;At the same time,it can reduce the level of postoperative insulin resistance. |
