Clinical Effect Of Unilateral Vidian Neurectomy For Allergic Rhinitis And Analysis Of Related Influencing Factors

Objective:To investigate the clinical effect of unilateral vidian neurectomy on allergic rhinitis(AR)and analyze the related factors that affecting the clinical effect,and to provide a certain basis for clinical treatment and the judgement of surgical prognosis.Methods:A total of 130 patients with AR admitted to the Second Hospital of Shanxi Medical University from January 2013 to January 2017 were selected as the study objects,all of whom were treated with unilateral vidian neurectomy.Preoperative CT scan was pe rformed,based on the anatomical classification of pterygoid nerve,By reading the CT sca-n before surgery,based on the anatomical classification of pterygoid nerve,and taking th-e simultaneous opening of the sinusesfor the combination as a reference,the operative si de was carefully selected.And all patients underwent unilateral pterygoid nerve resectio n under nasal endoscope.With these as the research objects,relevant data of 130 patients with AR were collected,including age,gender,length of stay,smoking,dust mite allergy,pollen allergy,nasal polyps,asthma,Visual analogue scale(VAS)to evaluate the clinical treatment effect after one-year following-up.According to the subjective feelings of patie nts,the clinical efficacy of 1 year after the operation was evaluated,which was divided in to obvious effect(disappearance of symptoms and signs),effective(symptoms and signs were alleviated compared with VN before the operation,but drug adjuvant treatment was needed)and ineffective(symptoms and signs were not significantly changed or even agg-ravated compared with VN before the operation).According to the VAS score one year after the operation,it was divided into the effective group(the VAS score after the operation was 0 or the VAS score changed ≧3).The invalid group(no significant change in postoperative VAS score compared with that before surgery).At the same time,the method of Logistic regression was used to analyze the influencing factors of clinical effects.Results:Of all the 130 patients with AR,the rate of significant improvement was up to79.23%(103/130).The effective rate was reached 13.85%(18/130).The inefficiency rate was 6.92%(9/130).Logistic regression showed that the main factor affecting the clinical effect was the combination of the nasal polyps.Others factors including age,gender,length of stay,smoking,dust mite allergy,pollen allergy,asthma,preperative scoring VAS wouldn’t influence the effects.Conclusion:The unilateral vidian neurectomy for the treatment of AR can achieve a comparatively better clinical effect.However,allergic rhinitis with nasal polyps showed a poor prognosis.
