Objective : Clinical observation on the treatment of choledocholithiasis patients with duodenal papillary sphincterotomy(EST)combined with papillary columnar balloon dilatation(EPBD),To study the incidence of pancreatitis and other complications after EPBD at different times(immediate termination,30 s,1min,3min and 5min),Objective: to investigate the optimal duration of balloon dilatation in small duodenal incision with columnar balloon dilatation.Methods:Patients with choledocholiths treated by EPBD after small incision were collected from the general hospital of ningxia medical university from January 2015 to December 2018.Patients who met the inclusion criteria were assigned to different groups of 0s,30 s,1min,3min and 5min according to the different duration of balloon dilation after biliary intubation.The probability of postoperative complications was analyzed through statistics,and SPSS22.0statistical software was used for statistical analysis of data in each group.The statistical description of measurement data is adopted,and the statistical description of counting data is adopted;The enumeration data were compared by test,To study the effect of the length of time of cylindrical balloon dilation on postoperative complications during EST combined with EPBD.Results:A total of 295 patients with choledocholithiasis treated by EPBD and small incision were collected from the general hospital of ningxia medical university from January 2015 to December 2018.These 295 patients who met the inclusion criteria were included in the modified intentionality treatment analysis(0s[n=76],30s[n=59],1min[n=68],3min[n=46],5min[n=46]).Overall,postoperative pancreatitis occurred in 43(14.6%)of the 295 patients treated by subgroup,One(0 s group of 13 cases(18.4%),30 s group and 1 case(1.7%),1 min group of 13 cases(19.1%),3 min group of 9 cases(19.6%),5 min group of 8 patients [17.4%]).The incidence of pancreatitis after ERCP was significantly higher in the groups of 0s,1min,3min and 5min(p=0.016).There was no significant difference in the stone extraction rates of each group(all >=90%).Gastrointestinal bleeding,perforation and cholangitis were not statistically significant in 295 patients treated with ERCP.Conclusion : Endoscopic sphincterotomy combined with balloon dilatation for 30 s can significantly reduce the incidence of pancreatitis after ERCP,which is the best time for balloon dilatation in the treatment of common bile duct stones.
