Objective:1.To analyze the differential expression of serum CEA,AFP,CA199 and CA724 in patients with proximal and distal gastric cancer in Qinghai;2.Analyze the correlation between the above markers and clinical case characteristics, and discuss the biological differences and clinical significance of the occurrence and development of gastric cancer in different sites;3.To analyze the differential expression of P53 and Ki67 in patients with proximal and distal gastric cancer in Qinghai. Methods:This study collected 149 patients with gastric cancer diagnosed in affiliated hospital of Qinghai university from December 2018 to April 2019,among whom 67 cases were proximal gastric cancer and 82 cases were distal gastric cancer. To analyze the differential expression of serum CEA,AFP,CA199 and CA724 in the PGC and DGC groups and their correlation with clinical characteristics,as well as the differential expression of P53 and Ki67 in patients with proximal and distal gastric cancer.Electrochemiluminescence immunoassay(ECLIA)and Chemiluminescent immunoassay(CLIA)?was used to detect tumor markers;Ki67 and P53 expression were detected by immunohistochemistry(ElivisionTMplus)method, and chi-square test, continuous correction method and Fisher were used in statistics. The test level was equal to 0.05.Results: 1.There was no significant difference in age, gender and nationality in the comparison of the general data of the proximal gastric cancer group and the distal gastric cancer group, which was statistically insignificant(P > 0.05).However,most of the patients in the proximal gastric cancer group were male, han nationality and > 60 years old;The majority of patients in the distal gastric cancer group were female, han nationality and aged ≤60 years.2.The expressions of CEA,AFP,CA199 and CA724 showed no significant differences in proximal gastric cancer group and distal gastric cancer group(P > 0.05).3.There was no difference between proximal gastric cancer group and distal gastric cancer group in T stage,N stage and?Pathology?type(P >0.05).However,it was statistically significant in TNM stage,Lauren classification and differentiation degree(P<0.05,P values are 0.043 0.030 0.036,respectively).Proximal gastric cancer was characterized by low differentiation, intestinal type and stage III;Distal gastric cancer was characterized by low, medium differentiation, diffuse type, stage I and II.It is suggested that proximal gastric cancer is characterized by poor differentiation and late disease stage, which reflects the lack of early diagnosis and rapid disease progression of proximal gastric cancer.4.There was no significant difference between P53 and Ki67 in proximal gastric cancer group and distal gastric cancer group(P > 0.05).5.Correlation analysis of CEA,AFP,CA199,CA724 expression and clinicopathological characteristics, there were no differences in gender, nationality, pathological type,P53 and Ki67(P > 0.05).But,the positive expressions of CEA,CA199 and CA724 were correlated with T stage,N stage and TNM stage, respectively; The positive expression of CEA and CA199 was correlated with the degree of differentiation; The positive expression of CEA was correlated with age;The positive expression of CA199 was associated with Lauren type,The deeper the infiltration of gastric cancer, the higher the stage, and the positive lymph node metastasis, the higher the positive expression rate of CEA,CA199,CA724.Conclusions: 1.In the correlation analysis between the expressions of CEA,AFP,CA199 and CA724 and the clinicopathological characteristics, the positive expressions of CEA,CA199 and CA724 were correlated with T stage,N stage and TNM stage, respectively; The positive expression of CEA and CA199 was correlated with the degree of differentiation; The positive expression of CEA was correlated with age;The positive expression of CA199 was associated with Lauren type.2.In the comparison between the two groups of proximal and distal gastric cancer, there were differences in TNM stage,Lauren classification and degree of differentiation.3.In other clinical case characteristics comparison,There were no significant differences in age,gender,nationality,CEA,AFP,CA199,CA724 expression, pathological type,T stage,N stage,P53,Ki67 between the proximal gastric cancer group and the distal gastric cancer group; The expressions of CEA,AFP,ca199 and ca724 were not significantly different from the pathological types,P53 and Ki67.
