| Objective:The research on oval foramen(PFO)and cryptogenic stroke(CS)in young and middle-aged people is mostly in the plain area.Currently,no research has been conducted in high-altitude hypoxic areas. This study explores PFO in high-altitude hypoxic environments. This will lead to the incidence of CS in young and middle-aged people,in order to provide a certain theoretical basis for the prevention and treatment of CS in young and middle-aged patients. Methods:This study screened 156 unexplained ischemic stroke patients who were admitted to Qinghai Provincial People’s Hospital from January 2013 to March 2019 and have lived in Qinghai for generations or have lived for more than 10 years,95 were men and 61 were women, aged 33 to 55 years. At the same time,160 non-stroke people matching the study group were selected as the control group at the medical examination center, including 82 males and 78 females.According to the results of cardiac color ultrasound(TTE)and acoustic contrast imaging(c-TTE),the study group and the control group were divided into PFO group and non-PFO group. The PFO diameter and right-to-left partial flow were observed, and the general data of the patients were recorded.,Cerebrovascular disease-related risk factors,National Institutes of Health Stroke Scale(NIHSS)score at admission and discharge, and then statistical analysis. Results:The research group showed that the detection rate of PFO was 46.1%(72/156),the average PFO diameter was(2.2 ± 0.45)mm,and the proportion of middle and large right-to-left shunts(RLS)was 59.7%(43/72),which was significant The detection rate was 26.8%(43/160)higher than that of the control group, and the PFO diameter was small and the RLS was small. The average diameter was(1.0 ± 0.38)mm,and the proportion of medium and large RLS was 25.8%(11/43),There were statistically significant differences in PFO diameter, detection rate, and different subflows between the two groups(P <0.05);There was a(weak)positive correlation between the degree of shunt in the study group and the size of the NIHSS score(correlation coefficient was 0.246,P = 0.037),and there was no significant correlation between the different PFO diameters and the size of the NIHSS score(P> 0.05);the combination of PFO in the study group with hypertension, diabetes, coronary heart disease,Homocysteine(Homocysteine,HCY),carotid atherosclerosis, hyperlipidemia and long-term smokers(smoking history),long-term drinkers(history of drinking)were not significantly different from those in the PFO group.Significance(P> 0.05).Conclusions: 1.There is a correlation between cryptogenic stroke and open oval foramen in young and middle-aged people in Qinghai,The incidence of PFO in patients with cryptogenic stroke is higher than that in the general population.2.In addition to the identified cerebrovascular risk factors,PFO in Qinghai can be another important risk factor for ischemic stroke.3.The younger and middle-aged patients with larger diameter of RLS and PFO in PFO patients in Qinghai are more likely to have a stroke. |
