| Objective: To explore the diagnosis value of serum CCL18 and other common laboratory biomarkers for diagnosis AECOPD with pneumonia,and evaluate relevant clinical significanceMethods:A total of 131 patients with AECOPD were in included and divided into AECOPD group and AECOPD + CAP group according to imaging results.Venous blood was collected within 24 hours of admission,and serum CCL18 levels were detected.Demographic data,prognostic indicators,CCL18 and others biomarker were compared between the two groups.Analyze the correlation between CCL18 and other indicators.Evaluate the diagnostic efficacy of CCL18,other common indicators and their combined indicators on AECOPD + CAP.Results: Serum CCL18 was significantly higher in the AECOPD group than in the AECOPD + CAP group(p <0.001),and WBC,Neu,Neu%,NLR,and CRP were higher in the AECOPD + CAP group than in the AECOPD group(p <0.05).Serum CCL18 was positively correlated with FEV1(r =(0.173,p = 0.048)and FVC(r = 0.174,p = 0.047);The AUC for serum CCL18 diagnosis AECOPD + CAP was 0.795,the AUC of CCL18 was the largest in single biomaker,and the diagnostic efficacy of CCL18 was higher than Neu,Neu%(p <0.05),similar to NLR and CRP.The combined indicator established by CCL18,NLR,and CRP has the largest AUC for the diagnosis of AECOPD + CAP,the AUC is 0.861,the sensitivity is 81.25%,and the specificity is 79.10%.Conclusion: Serum CCL18,WBC,Neu,Neu%,NLR,and CRP have a certain value in diagnosing AECOPD with pneumonia.Combining CCL18,CRP,and NLR can improve the diagnostic efficacy of AECOPD with pneumonia. |
