ObjectiveTo explore the association between+45T/G and+276G/T polymorphisms of adiponectin gene,physical activity and the risk of Metabolic Syndrome(MS)in adults of Nanjing,China.To analyze the association between+45T/G and+276G/T polymorphisms of adiponectin gene,physical activity and the risk of MS related clinical indicators in adults of Nanjing,China.To analyze the effects of the interaction between adiponectin gene polymorphisms and physical activity on MS and related clinical indicators in order to provide the theoretical basis for the prevention and etiological research of MS.Method(1)The subjects of the present study were from the 2010-2013"Survey on Nutrition and Related Health Status of Urban and Rural Residents in Nanjing".The multi-stage stratified and population-proportioned group random sampling method was used to extract the survey subjects.Questionnaire,physical examination and laboratory examinations were conducted to collect the basic characteristics including the sex,date of birth,occupation,etc.;personal health status such as smoking and drinking;physical activity;baseline characteristics such as height,weight,waist circumference,blood pressure,fasting plasma glucose(FPG),total cholesterol(TC),triglyceride(TG),high density lipoprotein cholesterol(HDL-C).According to the inclusion and exclusion criteria,a total of 226 eligible MS patients were included in the case group,and 183 healthy persons were included in the control group.No significant difference was observed between two groups in age and gender distribution.(2)The DNA of the subjects were extracted by the TIANamp blood DNA kit.Genotyping of the adiponectin gene+45T/G and+276G/T polymorphisms were performed by polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP).The distributions of genotypic and allelic frequencies among two groups were compared by chi-square test,and the corresponding risk was calculated using binary logistic regression.(3)Physical activity was assigned by metabolic equivalents(METs).The association between different types of physical activity intensity,physical activity levels,and total physical activity with the risk of MS and related indicators were analyzed.(4)Additive scales statistical model was used to explore the interactions between gene polymorphisms and physical activity to observe their effects on the risk of MS as well as related clinical indicators.Results(1)We found polymorphisms in the+45T/G loci and+276G/T loci of adiponectin gene among adult residents in Nanjing.The distribution of+45T/G loci and+276G/T loci genotypes were consistent with Hardy-Weinberg equilibrium law(P>0.05).(2)No significant difference was found in the distribution of the genotypic(χ~2=0.994,P=0.608)and allelic frequencies(χ~2=0.859,P=0.608)of+45T/G polymorphism between MS case and healthy control groups.No significant association was found in the codominant,dominant,and recessive models with the risk of MS(P>0.05).No association was found between+45T/G polymorphism and MS related indicators.(3)There was a significant difference in the distribution of genotypic(χ~2=6.766,P=0.034)and allelic frequencies(χ~2=5.935,P=0.015)of the+276G/T polymorphism between the MS case and the healthy control groups.Gender stratification analysis found that this difference only existed in males(P<0.05)but not in females(P>0.05).The TT genotype(OR=0.42,95%CI:0.19-0.91)was a protective factor for MS in adult residents of Nanjing,and the GG genotype(OR=1.57,95%CI:1.04-2.37)was a risk factor for MS in adult residents of Nanjing.After gender stratification analysis,GT(OR=0.47,95%CI:0.23-0.96)and TT(OR=0.31,95%CI:0.10-0.96)genotypes were protective factors for MS only in male adults.The GG genotype was a risk factor for MS in male adults(OR=2.26,95%CI:1.13-4.52).The+276G/T polymorphism was not associated with MS in females in Nanjing(P>0.05).+276G/T polymorphism was associated with MS related indicators except HDL-C.(4)No statistically significant differences in the occupational,traffic,leisure physical activity intensity and levels and sedentary behavior distribution between the MS case group and the healthy control group were observed(P>0.05).Univariate logistic regression analysis found that the intensity and levels of occupational physical activity,traffic physical activity,leisure physical activity as well as static behavior were independent of MS(P>0.05).After adjusting confounders,the results indicates that high-intensity leisure physical activity was a protective factor for MS(OR=0.47,95%CI:0.24-0.95).(5)The total physical activity was significantly different between MS case and healthy control groups(P<0.05).Univariate and multivariate logistic regression analysis found that moderate(OR~a=0.33,95%CI:0.13-0.81;OR~b=0.32,95%CI:0.12-0.82)and high levels(OR~a=0.38,95%CI:0.17-0.87;OR~b=0.34,95%CI:0.14-0.83)of physical activity were associated with lower risk of MS.Moderate and high levels of physical activity were found to be protective factors of waist circumference and HDL-C,moderate total physical activity was associated with lower FPG level and high total physical activity was found to be a protective factor of TG and systolic blood pressure levels(P<0.05).(6)No interaction was found between the apM1 gene+276G/T polymorphism and physical activity on MS risk.Conclusion(1)The present study indicates the+45T/G polymorphism of adiponectin gene was not associated with MS risk in adult residents in Nanjing,China.+276G/T polymorphism of adiponectin gene was a susceptibility site for MS in adult residents in Nanjing.The TT genotype and T allele of+276G/T polymorphism were protective factors among adult residents in Nanjing,however gender differences in this association may affect the results.Besides,+276G/T polymorphism was associated with MS related indicators except HDL-C.(2)Different levels of occupational physical activity and traffic physical activity were not related to the risk of MS,but high-intensity leisure-time physical activity was a protective factor of MS;different levels of occupational physical activity,traffic physical activity and leisure-time physical activity were not associated with the risk of MS;sedentary behavior was not associated with MS risk;moderate and high levels of physical activity were protective factors for MS.Moderate and high levels of physical activity were correlated with waist circumference,TG,HDL-C,FPG,and systolic blood pressure levels.(3)No interaction was found between the apM1 gene+276G/T polymorphism and physical activity on MS risk.
