Objective: The function of knee joint after total knee arthroplasty was compared with two methods,and the clinical effect of the two methods was analyzed.Methods: A total knee arthroplasty was performed during January 2017~ December2018 to treat osteoarthritis of the knee.60 patients were divided into study group and control group according to the different methods of using lower extremity to line.the study group used kinematics to guide total knee replacement,and the control group used traditional mechanical mechanics to guide total knee replacement.The operative time,hospitalization time,hospitalization cost,the lateral angle of the distal femur before and after operation,the medial angle of the proximal tibia before and after operation,the direction angle of the knee joint before and after operation,the KSS score of June and December before and after operation,the WOMAC score,the movement of the knee joint,the walking distance of 2 minutes,and the postoperative complications occurred at the last follow-up.Statistical analysis.Results: There was no significant difference in sex,age,BMI 、 follow-up time between the two groups in this study(P>0.05).operation time in the study group was longer than that in the control group(P=0.044).average length of hospitalization in the study group was longer than that in the control group(P=0.033).and there was no statistical difference in hospitalization expenses between the two groups(P=0.441).There was no significant difference in KSS score,knee motion and WOMAC、m LDFA、m MPTA、JLOA between the two groups before operation(P>0.05).the study group had an average postoperative KSS score(69.3 ± 7.59),the control group had an average postoperative KSS score(65.57 ± 4.72).the study group had a better postoperative june KSS score than the control group,and the difference was statistically significant(P=0.026).The mean KSS score(90.68 ± 3.34)in the study group and the mean KSS score(88.6±4.27)in the control group were better than those in the control group.The difference was statistically significant(P=0.039).the study group had 96.2±12.17 degrees of knee joint activity after operation,and the control group had 89.00±10.82 degrees of knee joint activity after operation.the study group had better knee joint activity after 6 months than the control group,and the difference was statistically significant(P=0.019).the study group had 113.5 ± 7.28 degrees of knee joint activity after operation,and the control group had 109.5±6.72 degrees of knee joint activity after operation.the study group had better knee joint activity after12 months than the control group,and the difference was statistically significant(P=0.031).The average WOMAC score of the study group was 25.56±2.51,and that of the control group was 27.32±5.28.There was no significant difference in the score of June WOMAC between the two groups,(P=0.125).The mean WOMAC score of 12 months after operation in the study group was 22.33±5.57,and the mean WOMAC score of 12 months after operation in the control group was 24.17±5.79.There was no statistical significance in the two groups after operation,(P=0.215).The study group was 132.1±35.7 m walking distance after 2 June,the control group was 142.3±30.9m walking distance after 2 June,and the two groups were not statistically significant(P=0.242).the study group had a 2-minute walking distance of 157.6±66.1 m after surgery,and the control group had a 2-minute walking distance of 146.7±51.2 m after surgery.there was no statistical significance in the 2-minute walking distance of 12 months after surgery,(P=0.478).Postoperative complications occurred in 3 cases(1case of lower extremity intermuscular venous thrombosis and 2 cases of incision exudate),and in 4 cases(1 case of lower extremity intermuscular venous thrombosis,1case of incision exudate and 2 cases of anterior knee pain).and the incidence of postoperative complications was not statistically significant in both groups(P=0.688).Conclusion: with short-term studies,KA group had better results in terms of KSS score and knee motion than MA group;current short-term clinical evidence suggests that KA technique is a safe and reliable alignment technique,and relative MA does not increase surgical complications.
