Objective:To evaluate the effect of tracheal extubation in the lateral position of 15°head high slope on the respiratory function of obese patients during general anesthesia recovery.Methods;One hundred and twenty patients underwent general anesthesia surgery,62males and 58 females,age 18-65 years,BMI?28 kg/m2,ASA physical status I or II.The random number table method was divided into 4 groups?n=30?:supine position group?group C?,15°head high slope supine position group?S group?,15°head high slope left lateral position group?L group?and 15°head high slope right side?R group?.All patients underwent surgery under general anesthesia.In group C,supine position was used,group S used 15°head high slope supine position,group L used15°head height slope left side,and group R used 15°head height slope right side.Record SpO2?PaO2?MAP?HR at the time of entry?T0??1min?T1??5min?T2??30min?T3?and 2h?T4?after extubation;The time to remove tracheal tube and the time to leave the PACU were recorded.Adverse reactions such as cough?sore throat?laryngeal spasm and tongue drop were recorded.Results:Compared with T0,SpO2 was significantly decreased in 4 groups of T1-4?P<0.05?,MAP and HR were significantly increased at T1-2?P<0.05?;Compared with group C,SpO2?PaO2 in group S,group R and group L at T1-4-4 was significantly increased?P<0.05?,MAP?HR at T1-2-2 was significantly decreased?P<0.05?,The time to remove tracheal tube and the time to leave the PACU were significantly shortened?P<0.05?.The incidence of tongue drop and cough in R group and L group were significantly reduced?P<0.05?.Compared with the S group,SpO2 and PaO2 in the R group and the L group were significantly increased?P<0.05?,The time to remove tracheal tube and the time to leave the PACU were significantly shortened?P<0.05?,the incidence of tongue drop and cough was significantly lower?P<0.05?;There was no statistically significant difference in the incidence of pharyngeal pain and laryngospasm in the 4 groups.Conclusion:The tracheal extubation in the lateral position of the 15°head high slope can improve the oxygen reserve during the recovery period of obese patients and promote the recovery of respiratory function.
