| Objective:In this study,the heat sensitive moxibustion combined with auricular acupoint pressing for the anterior cruciate ligament reconstruction(ACL-R)postoperative patients was used to conduct Chinese medicine nursing intervention,taking pain,sleep quality,anxiety and calm adverse reactions occurring in the process of pain as the corresponding observation indicators,and striving to explore more effective external treatment techniques of traditional Chinese medicine nursing to provide a basis for pain management after knee ACL-R patientsMethod:This trial is a type of randomized controlled trial.The subjects of the study by random sampling were patients with knee ACL-R who were admitted to the Department of Sports Medicine at a third class general hospital in Lanzhou City,Gansu Province from September 2018 to November 2019.140 patients were selected according to the criteria of nanoplating.The patients were divided into 4 groups by random number table method:control group(conventional treatment),treatment group I(heat sensitive moxibustion group),treatment group II(ear acupoint pressing beans group),treatment group III(heat sensitive moxibustion combined with ear acupoint pressing beans group),35 people in each group.The ear acupoint pressure beans are mainly based on Shenmen,Sympathy,and subcortical points,and are related to the organs with knee ankle points,once a day,once for 15 to 20 minutes,and a course of treatment for 5 days.The heat sensitive moxibustion uses the pure moxa stick,which is ignited The heat sensitive acupoints are detected within the radius of 3 cm with the meridian points,tenderness points,subcutaneous hard nodes and other parts near the affected knee as the center It is determined that the most intense heat sensitive moxibustion sensation is the sensitized acupoint.The warm moxibustion is conducted 2cm from the skin until the moxibustion sensation disappears,once a day and once a course of 5 days,a total course of treatment.The patients’ pain,sleep quality,and anxiety were evaluated before the first postoperative treatment,after the treatment,and after the postoperative daily treatment.Short version of McGill pain questio-naire(SF-MPQ),Athens Insomnia Scale(AIS),and Self-Rating Anxiety Scale(SAS)were used for scoring Scale.In addition,before the first postoperative treatment and after the last treatment,the patients were scored using the inpatient care satisfaction scale,and the hospitalization time,adverse reactions and influencing factors of all patients were recorded.In the study,SF-MPQ was used as the main score for pain care evaluation,and AIS,SAS and inpatient care satisfaction were used as auxiliary scores.The four scales comprehensively reflected the degree of postoperative pain from different degrees.The four groups of data were collected and organized,and SPSS23.0 was used for data analysisResults:1.Comparison of SF-MPQ scores:Overall comparison,there were differences in SF-MPQ scores among patients in different groups and at different treatment time points(P<0.05).The scores of SFMPQ scale at different time points after treatment in each group were significantly lower than those before treatment(P<0.05).The overall comparison between different groups in the same treatment stage,different clinical treatment methods have different clinical effects(P<0.05).The combination of heat-sensitive moxibustion and auricular pressure acupuncture is superior to other groups in analgesia after ACL-R operation(P<0.05);the short-term clinical effects of heat sensitive moxibustion and auricular pressure acupressure therapy after ACL-R operation are equivalent(P<0.05)2.Comparison of AIS scores of four groups of patients:Comparison between the groups found that there are differences between the four therapies in each treatment time period(P<0.05),and heat-sensitive moxibustion combined with ear acupuncture has certain advantages in treating sleep disorders after ACL-R operation(P<0.05).Within the group comparison,the four treatment methods all improved the sleep disturbance of the patients at the time point after the first treatment after ACL-R operation(P<0.05).The short-term clinical effect of heat sensitive moxibustion combined with auricular point pressing bean in improving postoperative sleep of patients was more significant than that of conventional therapy and single therapy(P<0.05)3.Comparison of the SAS scale scores of the four groups of patients:There were differences for the four groups of patients alleviated their anxiety after ACL-R at different time points and different treatment methods(P<0.05);the overall comparison of the four groups of patients at each time there were differences in point SAS scores(P<0.05);the overall comparison between the four therapies showed differences in the degree of anxiety of patients with clinical relief(P<0.05);the comparison between the two groups of heat sensitive moxibustion combined with auricular acupressure compared with heat-sensitive moxibustion,the short-term effect of improving postoperative anxiety in patients is more significant(P<0.05)The effect of heat sensitive moxibustion and ear acupressure on the anxiety of patients with postoperative patients tends to be the same with the extension of treatment time(P>0.05);the comparison within the group shows that the four treatment methods have a significant clinical effect on improving patient anxiety after ACL-R(P<0.05)4.Comparison of the nursing satisfaction scores of the four groups of patients:After comparison,the nursing satisfaction between the four treatment methods was statistically significant(P<0.05).The final nursing satisfaction of the four groups of patients after operation was improved compared with that before the operation(P<0.05).There were differences in nursing satisfaction among the groups after the last postoperative treatment,and the heat sensitive moxibustion combined with auricular pressure acupuncture,heat sensitive moxibustion,and auricular pressure acupressure all improved the patient’s nursing satisfaction,and the combined group had more advantages(P<0.05)5.Comparison of hospital stays in four groups:There was no statistically significant difference in the distribution of hospital stays among the four groups(P>0.05).At the same time,all patients were hospitalized for more than 5 days,mainly between 8 days and 13 days According to the interquartile spacing of the four sets of data,the degree of dispersion does not change much6.Safety evaluation:During the course of treatment,the condition and vital signs of the four groups of patients were stable.In the control group,heat-sensitive moxibustion and combination group,3 cases,3 cases and 4 cases showed symptoms such as nausea and poor appetite after operation.The symptoms appeared within 24 hours after operation.There were no obvious adverse reactions in the acupoint group and the pressure bean group7.Analysis of influencing factors of curative effect:The curative effect of the four groups was not related to the surgical experience,postoperative pain experience,postoperative pain level,anesthesia mode,smoking and alcoholism(P>0.05);the operation time and the first pain time after anesthesia are also irrelative(P>0.05)Conclusions:1.Heat sensitive moxibustion combined with auricular acupoint pressing beans has obvious curative effects in analgesic care after knee ACL-R,and can effectively relieve pain symptoms2.Heat sensitive moxibustion combined with auricular acupoint pressing can alleviate the symptoms of anxiety and insomnia after ACL-R of the knee joint,and it has certain value as a supplementary observation index for analgesic care3.Heat sensitive moxibustion combined with auricular acupoint pressing can increase the nursing satisfaction of patients after ACL-R surgery,and can also be used as a supplementary observation index for postoperative nursing analgesia4.Heat sensitive moxibustion and auricular point pressing beans also have certain clinical effects in nursing analgesia after ACL-R,and can be used as auxiliary methods in clinical pain nursing after surgery5.Auricular acupoint pressing and heat sensitive moxibustion therapy have the effects of improving sleep,varying degrees of pain,and postoperative satisfaction of patients after ACL-R.Especially,heat sensitive moxibustion has unique effects in improving anxiety. |
