| [Objective] Objective to investigate the changes in serum levels and clinical significance of brain-derived neurotrophic factor(BDNF)in type 2 diabetes mellitus(T2DM),acute cerebral infarction(ACI),and T2 DM complicated with ACI.[Method] According to the Chinese 2017 version of the prevention and treatment guidelines for type 2 diabetes mellitus and the diagnostic criteria in the Chinese 2018 edition of the guidelines for the diagnosis and treatment of acute ischemic stroke,102 subjects who met the criteria were selected from the Department of Endocrinology and neurology of the second hospital of Lanzhou University from June 2019 to December 2019,and 30 healthy subjects were selected from the physical examination center at the same time.Their general clinical data were collected and blood pressure,blood glucose,blood lipid and glycosylated hemoglobin were measured.According to the disease,the subjects were divided into 4 groups:(1)T2DM complicated with ACI group: 33 cases;(2)simple T2 DM group: 36 cases;(3)simple ACI group: 33 cases;(4)normal control group: 30 cases.Their serum levels of BDNF were detected by enzyme-linked immunosorbent assay(ELISA).All data were analyzed by SPSS 23.0.The difference was statistically significant when P<0.05.[Result] 1.One-way analysis of variance results: the serum BDNF level in T2 DM group was significantly lower than that in normal control group(P<0.05).The serum BDNF level in ACI group was significantly higher than that in normal control group(P<0.05).The serum BDNF level in T2 DM complicated with ACI group was significantly higher than that in simple T2 DM group(P<0.05);serum BDNF level in T2 DM complicated with ACI group was significantly lower than that in ACI group(P<0.05);there was no significant difference between T2 DM complicated with ACI group and normal control group(P>0.05).2.Pearson correlation analysis results: In the analysis of simple T2 DM group and normal control group,BDNF was negatively correlated with DBP(R =-0.360),TC(R =-0.334),Hb A1c(R =-0.366)(P<0.05),but not with SBP,TG,HDL and LDL(P > 0.05).In the analysis of simple ACI group and normal control group,BDNF was positively correlated with TC(R = 0.409)and LDL(R = 0.355)(P<0.05),but not with SBP,DBP,TC,TG,HDL and Hb A1c(P>0.05).In the analysis of four grougs,there was no correlation between BDNF and each component(P>0.05).4.Binary logistic regression analysis results: serum BDNF was a protective factor in the diagnosis of T2DM(P<0.05);serum BDNF was a risk factor for the diagnosis of ACI(P<0.05);serum BDNF could not be used as an influencing factor in the diagnosis of T2 DM complicated with ACI(P>0.05).[Conclusion] 1.The serum BDNF level of T2 DM group was lower than that of normal control group;the serum BDNF level of ACI group was higher than that of normal control group;the serum BDNF level of T2 DM complicated with ACI group was higher than that of T2 DM group,but lower than ACI group,and there was no significant difference between T2 DM complicated with ACI group and normal control group.2.BDNF may participate in the occurrence and development of T2 DM by affecting blood lipid,blood pressure and blood glucose,and BDNF may participate in the occurrence and development of ACI by affecting blood lipid. |
