Objectives1.To explore the effects of nursing intervention based on the Integrated Theory of Health Behavior Change on compliance of functional exercise,self-efficacy of rehabilitation, quality of life, knee activity and knee function of patients undergoing total knee replacement.Methods1.In strict accordance with the inclusion and exclusion criteria,33 patients proposed to undergo total knee arthroplasty in the joint surgery ward of a third class a general hospital in Tianjin from June 5,2019 to July 5,2019 were selected as the control group, and 33 patients proposed to undergo total knee arthroplasty in the joint surgery ward of a third class a general hospital in Tianjin from July 25,2019 to August 25,2019 were selected as the intervention group. The control group was given routine nursing intervention, and the intervention group was given health education based on the theory of health behavior change integration. The subjects were measured by SER,rehabilitation training compliance scale,SF-12,knee joint activity and KSS before intervention,4 weeks and 12 weeks of intervention.2.The data was analyzed with SPSS 21.0 statistical software,The main statistical methods used were descriptive analysis,chi-square test,t-test,repeated measures analysis of variance. With a=0.05(bilateral)as the test level,a significance difference was defined as P<0.05.ResultsFinally,a total of 63 patients completed this study(32 in the intervention group and 31 in the control group),1 in the intervention group due to loss of follow-up,2 in the control group,1 in the active withdrawal and 1 in the loss of follow-up.Before the intervention, there were no significant differences in the general demographic data,disease-related data, rehabilitation training compliance, rehabilitation self-efficacy score, quality of life score,KSS score and knee joint activity between the two groups(P>0.05).At 4 weeks of intervention, the compliance of rehabilitation training in the intervention group was significantly higher than that in the control group(P<0.05).At the 12 th week ofintervention, the compliance of functional training in the intervention group was significantly higher than that in the control group(P<0.05).At 4 weeks of intervention, the rehabilitation self-efficacy score of the intervention group was(66.94±3.85),which was higher than that of the control group(64.52±3.86),and the difference was statistically significant(P<0.05).At the 12 th week of intervention, the rehabilitation self-efficacy score of the intervention group was(72.25±4.13),which was higher than that of the control group(66.61±4.27),the difference was statistically significant(P<0.05).After repeated measurement of variance analysis, the differences were statistically significant(P<0.05)between different time, group and interaction. At 4 weeks of intervention, the PCS and MCS scores of the intervention group were(51.09±7.48)and(65.10±5.57),which were slightly higher than the PCS and MCS scores of the control group(47.10±7.72)and(62.23±5.98),the difference was statistically significant(P<0.05).At the 12 th week of intervention, the scores of PCS and MCS in the intervention group were(59.84±7.57)and(69.92±5.77),and the scores of PCS and MCS in the control group were(53.06±6.79),(65.19±5.95).The difference was statistically significant(P<0.05);After repeated measurement of variance analysis, the differences were statistically significant(P<0.05)between different time, group and interaction. At 4 weeks of intervention, the KSS score of the intervention group was(106.78±17.94),which was higher than that of the control group(96.03±15.78),and the difference was statistically significant(P<0.05).At the 12 th week of intervention, the KSS score of the intervention group was(134.09±14.73),which was higher than that of the control group(124.65±13.82),and the difference was statistically significant(P<0.05).After repeated measurement of variance analysis, the differences were statistically significant(P<0.05)between different time, group and interaction. At 4 weeks of intervention, the knee joint activity of the intervention group was(104.38±7.49),which was higher than that of the control group(101.13±7.04),but the difference was not statistically significant(P>0.05).At the 12 th week of intervention, the knee joint activity of the intervention group was(113.91±8.00),which was higher than that of the control group(110.16±8.11),but the difference was not statistically significant(P>0.05).Afterrepeated measurement of variance analysis, the difference was statistically significant between different time(P<0.05),but there were no statistically significant differences between group and interaction.ConclusionsThe nursing intervention based on the theory of health behavior change integration can improve the self-efficacy of rehabilitation, the compliance of rehabilitation training and the function of knee joint of patients with total knee replacement.
