| Objective:Experiment 1Characteristics of gait changes in patients with unilateral KOA were analyzed,to observe the gait differences of KOA patients before and after treatment,to explore the effects of hot application of external washing bag and functional exercise on postoperative gait and surface myoelectricity in patients with kidney deficiency and blood stasis type KOAExperiment 2KDI 3D gait software was used to create a 3D dynamic model for 10 patients,To analyze the change of the maximum Angle of hip knee and ankle toe joint flexion before and after arthroscopic cleaning.Methods:Experiment 1From January 2019 to November 2019 are selected,Forty patients were admitted to the orthopedic department of the first affiliated hospital of tianjin university of traditional Chinese medicine for unilateral KOA arthroscopic,According to the visiting order,they were randomly divided into the experim ental group(20 cases)and the control group(20 cases).All the patients met the inclusion and exclusion criteria of the diagnostic criteria of traditional Chinese and western medicine Before treatment,the two groups went to tianjin orthopaedic institute of integrated traditional Chinese and western medicine for gait data collection,Arthroscopy was performed at our hospital,After operation,the experimental group received functional exercise combined with hot application of external washing package for 3 weeks,In the control group only functional exercise was performed,and data were collected again 3 months after arthroscopy.Experiment 2From October 2018 to January 2019,10 patients from the department of orthopedics of our hospital were admitted for treatment of unilateral knee osteoarthritis by arthroscopy.Before operation and 8 weeks after operation,they went to the orthopaedic research institute of Tianjin hospital.The 3D model was established by the NDI Optotrak Certus 3D motion measurement system and the Angle data of each joint of the gait were collected,To analyze whether the maximum flexion Angle of the ankle metatarsal joint of the affected side of the hip knee during walking after arthroscopic cleaning was different from that before.Result:Experiment 1A total of 40 patients were included in this study,all of whom had varying degrees of knee joint pain.Among the 20 patients in the test group,14 were female and 6 were male,The average age was 61.80±6.34 years,and the average course of disease was 40.10±47.99 months,There were 11 cases of left knee and 9 cases of right knee,The preoperative WOMAC score averaged 66.05±6.35;The control group included 15 females and 5 males,The average age was 61.10±9.46 years,The average course of disease was 38.20±48.90 months,There were 10 cases of left knee and 10 cases of right knee,The preoperative WOMAC score averaged 66.85±5.92.After treatment,the pain relief effect is significant,There was no significant difference in VAS WOMAC score between the two groups at 7 days and 90 days after surgery(P>0.05).,VAS WOMAC scores at 15 and 30 days after surgery showed significant difference(P<0.05).There were differences in bipedal gait between the two groups before treatment,The overall characteristics were that the percentage of the time of the lateral forefoot flat in the contact period was prolonged and the percentage of the forefoot impulse was increased,The pressure on the lateral side of the forefoot is higher than that on the right side,The integrated emg and the median frequency of the vastus medialis muscle of the rectus femoris lateralis are lower than that of the medial side.After treatment,the parameters of the lateral foot and the surface emg of the two groups were significantly changed compared with those before the operation,and the results of the paramet ers of the experim ental group were closer to the healthy side.Experiment 2The maximum flexion Angle of the ankle metatarsal joint of the affected side was significantly different from that of the operation(P<0.05).Conclusion:Experiment 11.Bipedal gait parameters of patients with unilateral KOA were different,and the endurance of quadriceps femoris of the affected side was significantly lower than that of the healthy side.2.Arthroscopic cleaning combined with functional exercise has a better therapeutic effect on KOA,and the auxiliary treatment effect with hot application of external washing bag is better than functional exercise alone.Experiment 2The maximal flexion Angle of the ankle metatarsals on the affected side of the hip and knee was different during walking in KOA patients.The maximal flexion Angle of each joint on the affected side after arthroscopic cleaning was improved compared with that before surgery,and the step length and step speed of the affected side were improved compared with that before surgery. |
