Objective To study the changes of T cell immune function and the intervention effect of Sijunzi Decoction in patients with post-hepatitis B cirrhosis and spleen deficiency syndrome,and provide clinical evidence for Ganbingshipi theory.Methods A phased clinical trial design was used.The first stage was designed by cross-sectional study.Patients with post-hepatitis B cirrhosis were divided into spleen deficiency syndrome group and non-spleen deficiency syndrome group.Flow cytometry combined with lymphatic five-test was used to evaluate the difference between the two groups in T cell immune function.The second stage was designed by paired controlled trials.The 1:1 pairing of patients with post-hepatitis B cirrhosis and spleen deficiency syndrome was divided into the experimental group and the control group.The control group was given basic treatment.The experimental group was given the Sijunzi decoction at the same time as the basic treatment.After 8 weeks of treatment,the efficacy indicators included changes in the scores of TCM spleen deficiency syndrome and T cell immune function.In the third part,protein chip technology was used to detect the serum protein expression of patients with s post-hepatitis B cirrhosis and spleen deficiency syndrome who treated by the Sijunzi decoction.The differentially expressed proteins were analyzed by GO analysis and KEGG enrichment analysis.Results1.In the first phase of the study,80 patients with post-hepatitis B cirrhosis were included.Compared with the spleen-deficiency group and the non-spleen deficiency group,the levels of CD3+ T cells,CD4+ T cells and CD8+ T cells were significantly decreased(P<0.05),and the expression of Treg was significantly increased.(P<0.05).2.In the second phase of the study,36 patients with post-hepatitis B cirrhosis and spleen deficiency syndrome were enrolled.After 8 weeks of treatment with Sijunzi Decoction,the scores of spleen deficiency syndrome before and after treatment in the experimental group were significantly more significant than those in the control group(P<0.05).The levels of CD3+ T cells,CD4+ T cells and CD8+ T cells weresignificantly higher than those before treatment,and the expression level of Treg was significantly decreased(P<0.05),which was significantly different from the control group(P<0.05).3.Three patients with post-hepatitis B cirrhosis and spleen deficiency syndrome treated with Sijunzi Decoction were tested for serum protein broad-spectrum screening antibody chips before and after treatment.It was found that Sijunzitang improved immune-related biological processes.Conclusions The abnormal immune function of T cells in patients with spleen deficiency syndrome after hepatitis B cirrhosis showed that the expression level of T lymphocyte subsets decreased and the expression level of Treg increased.Sijunzi Decoction can improve the biological process of acquired immune improvement such as Toll-like receptor signaling pathway and TGF-beta signaling pathway,and has a significant regulatory effect on T cell immune function,increasing the expression level of T lymphocyte subsets and decreasing the expression level of Treg,thus significantly improving patient's immune function. |
