Background Early neurological deterioration of acute cerebral infarction refers to the deterioration of neurological function within 72 hours after the onset of acute ischemic stroke,which is mainly manifested as the progressive aggravation of neurological defect symptoms and the worsening of the disease,which is obviously related to the disability and mortality of patients.25-hydroxyvitamin D3is one of the active forms of vitamin D3.Recent studies have shown that 25-hydroxyvitamin D3is negatively correlated with the severity of carotid atherosclerosis,and its deficiency or deficiency can increase the risk of cardiovascular disease.This study aimed to explore the correlation between serum25-hydroxyvitamin D3levels and the deterioration of neurological function in the early stage of acute cerebral infarction,in order to provide a certain reference basis for the early detection and treatment of neurological deterioration in the early stage of acute cerebral infarction.Objective To analyze the correlation between serum 25-hydroxyvitamin D3level and early neurological deterioration(END)in patients with acute cerebral infarction(ACI).Methods The cases of 131 patients with ACI were enrolled in this study from May 2018to May 2019.They were divided into END group(n=42)and non-END group(n=89)according to whether END occurred or not.The basic data of sex,age,smoking history,drinking history,disease history,NIHSS score,blood routine and laboratory biochemical indicators were collected.Serum 25-hydroxyvitamin D3levels were detected by ELISA in all patients.The clinical data of the two groups were compared and the correlation between serum 25-hydroxyvitamin D3 level and END in patients with acute cerebral infarction was analyzed by binary logistic regression model.Pearson correlation analysis was used to investigate the correlation between 25 hydroxyvitamin D3 level and hypersensitive C reactive protein and NIHSS score.Results END occurred in 42 of 131 patients with ACI,accounting for32.06%.Compared with the non-END group,the male ratio in END group was significantly lower,the age was significantly higher,the diabetes ratio and the coronary heart disease ratio were significantly higher,the NIHSS score was significantly higher,the serum levels of hypersensitive C-reactive protein was significantly higher,the serum levels of 25-hydroxyvitamin D3was significantly lower,the difference was statistically significant(P<0.05).However,there were no significant differences in smoking,drinking,hypertension,hyperlipidemia,white blood cell count,high density lipoprotein and low density lipoprotein between the two groups(P>0.05).Logistic regression analysis showed that the level of 25-hydroxyvitamin D3,diabetes mellitus were independent risk factors for END in patients with ACI.Pearson correlation analysis showed that serum 25-hydroxyvitamin D3level was negatively correlated with hypersensitive C reactive protein and NIHSS score.Conclusion The decrease of serum 25-hydroxyvitamin D3level is closely related to the occurrence of END in ACI.
