Objectives:Stroke is the first cause of death and disability in adults in China,and it has the characteristics of high morbidity,high disability,high mortality and high recurrence rate.At present,the injury mechanism of stroke is very complicated,and various factors are involved.The acupuncture effect is mainly achieved by inhibiting nerve cell apoptosis and promoting neural plasticity.At present,there is a lack of research on the effect mechanism of acupuncture intervention to promote neural plasticity,and the signal The pathway is not clear.Based on the cAMP / PKA-pCREB signaling pathway,this study explored the therapeutic effect of acupuncture "Baihui,Neiguan,Sanyinjiao" on neural function in rats with cerebral ischemia and the mechanism of neural plasticity recovery,provide an effective evidence-based basis for clinical acupuncture to promote functional rehabilitation after cerebral ischemic injury.Methods:Thirty-seven healthy SD rats that met the inclusion criteria were randomly divided into12 acupuncture groups,13 model groups,and 12 sham-operation groups according to the random number table method.The rats in model group and acupuncture group were modeled by middle cerebral artery occlusion and cerebral ischemia,and prepared by thread embolism method.Rats in the sham operation group and model group did not take intervention measures,and only performed the same time and the same degree of grasping control.In the acupuncture group,"Baihui" and "Neiguan" and "Sanyinjiao" of the affected side were taken.Baihui: puncture forward or backward by 2mm,twist and flatten and relieve(Xie 90-180 °,60-120 times / min)30 seconds,Neiguan point: straight puncture,needle insertion 1mm,large(angle> 180 °)rapid(> 120 times / min)twisting for 30 seconds,Sanyinjiao point:straight puncture,needle insertion 5mm,Twist with small amplitude(angle <90 °)and slow speed(<120 times / min)for 30 seconds,once a day,intervene 6 days a week,rest for 1 day,and continue treatment for 21 days.All the modeled rats were excluded according to the Zea Longa method.Each group of rats was evaluated with a modified neurological severity score(MNSS)at 24 h,7,14,and 21 days after surgery.Assess the neurological deficit of rats.The rats in each group were collected on the 7th,14 th,and 21 st day(left cerebral cortex tissue),and the expression of cAMP,PKA,CREB,and pCREB in the cerebral ischemic tissue were detected by Western blot,Elisa,and RT-PCR.All experimental data were created with SPSS21.0 software and analyzed statistically.Result:1.The MNSS scores of rats in each group at different time points showed that 24 h and 7days after operation: 24 h after operation: There was no statistical difference between the acupuncture group’s neurological deficits and the model group,indicating that before the acupuncture intervention,there was no significant difference between the acupuncture group and the model group’s neurological deficit status;7 Days and 21 days: There was no statistically significant difference in neurological deficits in the acupuncture group compared with the model group;14 days after surgery: There was a statistically significant difference in neurological deficits in the acupuncture group compared with the model group.At 14 days,compared with the acupuncture group,the model group had the lowest P value and the most significant value.2.The changes of cAMP content in the brain tissue of rats in different groups at different time points showed that:7 days,14 days,and 21 days after operation: the acupuncture group and model group had significant statistical differences in cAMP content in the brain tissue.From 7 days to 14 days,the cAMP content in the brain tissue of the acupuncture group showed a clear upward trend.On the 14 th day,the cAMP content in the brain tissue of the acupuncture group reached the highest level,while from 14 days to 21 days The content of cAMP in rat brain tissue began to decrease,suggesting that acupuncture can affect the expression of cAMP in ischemic brain tissue.3.The changes of PKA content in the brain tissue of rats in different groups at different time points showed that: At 7 days after surgery,the content of PKA in the brain tissue of the acupuncture group was statistically different from that of the model group;at 14 and 21 days after surgery,the content of PKA in the brain tissue of the acupuncture group was significantly different from that of the model group,Indicating that acupuncture can affect the content of PKA in rat brain tissue after cerebral ischemia.On days 7-14,the PKA contentin the brain tissue of the acupuncture group showed an increasing trend.On the 14 th day,the PKA content rose to the highest level,and from 14 days to 21 days,the PKA content in the brain tissue of the acupuncture group was obvious.The trend of decline is better than the model group.4.The changes in the content of pCREB in the brain tissue of rats in different groups at different time points showed that: 7 days after surgery: the ratio of pCREB in the brain tissue of the acupuncture group was not statistically different from that in the model group;14 days and 21 days after surgery,The ratio of pCREB in the brain tissue of the acupuncture group was significantly different from that of the model group;it showed that the acupuncture can affect the expression of pCREB protein in the rat brain tissue after cerebral ischemia.Conclusion:1.After successful modeling,the motor nerve function defects of the rats are obvious.Acupuncture "Baihui,Neiguan,Sanyinjiao" can significantly improve the nerve function of cerebral ischemia rats;2.Acupuncture "Baihui,Neiguan,Sanyinjiao" can promote the expression of cAMP,PKA,p-CREB protein in ischemic brain tissue,thereby activating cAMP/PKA-pCREB signaling pathway,which may be acupuncture to improve cerebral ischemia rats One of the mechanisms of nerve function.
