Discussion On Meridian Syndrome Differentiation Of Psoriasis Vulgaris Based On Distribution Of Skin Lesions

Objective:In this study,from the perspective of meridians,the main distribution sites of skin lesions in psoriasis vulgaris were observed,the distribution regularity of skin lesions in the twelve skins was analyzed,the common meridians of patients with psoriasis vulgaris were summarized,and the main symptoms of each meridian were summarized.Methods:The original data were collected in the form of questionnaire,and the skin lesions were recorded in the form of pictures and drawings.The general conditions,distribution of skin lesions and the skin where they were located,and the four diagnoses of traditional Chinese medicine of 213 patients with psoriasis vulgaris who met the inclusion criteria were collected and recorded in the "Psoriasis Patient Visit Information Collection Form." The frequency and percentage of skin lesions in the twelve skins at different sites were summarized,and the distribution rule of skin lesions in the twelve skins of patients with psoriasis vulgaris was summarized.According to the four diagnoses of symptoms of traditional Chinese medicine and other information,meridian syndrome differentiation was used to summarize the meridian syndrome differentiation rule of psoriasis vulgaris.Results:1.In this study,the top ten skin lesions were elbow(77.93%),lower leg(69.01%),back(65.26%),scalp(56.81%),lumbosacral(50.23%),abdomen(46.48%),thigh(31.92%),popliteal fossa(30.05%),groin(18.31%),and chest(17.37%).The least distributed site was the foot(6.57%).2.The skin lesions of the elbow were compared between different skin parts,2= 247.253,P < 0.05,with a statistically significant difference;among them,166 cases(91.21%)were most distributed in the Shaoyang Triple Jiao Meridian of the elbow.The skin lesions of lower leg were compared between different skin parts,2= 129.995,P < 0.05,with statistical difference;among them,111 cases(35.02%)were most distributed in the stomach meridian of Foot-Yangming in the lower leg.There was significant difference between different skin lesions on the back,2= 136.159,P < 0.05;88 cases(61.54%)were distributed most frequently in the bladder meridian of Foot-Taiyang on the back.The scalp lesions were compared between different skin parts,2 = 38.583,P < 0.05,with a statistically significant difference;among them,56 cases(43.75%)had the most Yanggan meridian in the scalp.3.In this study,the distribution of skin lesions in the twelve skins was the most distributed hand Shaoyang Triple Jiao Meridian,with a proportion of 81.22%;followed by Foot-Taiyang Bladder Meridian,with a proportion of 73.71%;Foot-Yangming Stomach Meridian again,with a proportion of 65.26%;the rest were Foot-Taiyin Spleen Meridian,with a proportion of 62.91%;Foot-Jueyin Liver Meridian,with a proportion of 56.81%;Foot-Shaoyang Gallbladder Meridian,with a proportion of 30.05%;Foot-Yangming Large Intestine Meridian,with a proportion of 25.82%;Hand-Jueyin Pericardium Meridian,with a proportion of 15.02%;Foot-Shaoyin Kidney Meridian,with a proportion of 14.55%;Hand-Taiyin Lung Meridian,with a proportion of 13.15%;4.The skin lesions mainly in the skin of Shaoyang triple energizer,the bladder skin of Foot-Taiyang and the stomach skin of Foot-Yangming were compared between different parts of the body surface,P < 0.05,with statistical differences.Among them,166 cases(82.18%)were distributed most frequently in the elbow,and the lesions were distributed in the elbow compared with the lesions distributed in the hand,forearm,upper arm,and back,P < 0.05,with statistical differences;88 cases(22.39%)were distributed most frequently in the back,and the lesions were distributed in the back compared with the lesions distributed in the foot,lower leg,popliteal fossa,thigh,buttock,and scalp,P < 0.05,with statistical differences,and the lesions distributed in the back compared with the lesions distributed in the lumbosacral region,2=1.308,P > 0.05,with no statistical differences;111 cases(40.51%)were distributed most frequently in the lower leg,and the lesions were distributed in the lower leg compared with the lesions distributed in the foot,knee,thigh,abdomen,and scalp,with statistical differences.5.According to the diagnostic criteria of syndrome differentiation of each meridian,the main contents of each meridian are summarized as follows:The main symptoms of hand Shaoyang triple energizer meridian syndrome in patients with psoriasis vulgaris are: skin lesions mostly distributed in the elbows,abdominal distension,tinnitus,constipation,and unfavorable urination.The main symptoms of Foot-Taiyang Bladder Meridian syndrome in patients with psoriasis vulgaris are: skin lesions are mostly distributed on the back and lumbosacral region,lower legs and popliteal fossa,irritability,head pain,low back pain,constipation,and night frequency.The main symptoms of foot Yangming stomach meridian syndrome in patients with psoriasis vulgaris are: skin lesions mostly distributed in the lower legs,poor appetite,abdominal cramps,thirst like to drink,breath filthy,nausea.The main symptoms of psoriasis vulgaris are: skin lesions mostly distributed in the head,bitter taste,hypochondriac pain,good rest,migraine.Conclusion: The main sites of psoriasis vulgaris lesions were elbows,lower legs,back,scalp,and lumbosacral region;the lesions were more distributed in the Shaoyang Triple Jiao skin of the hand,the Bladder skin of the Foot-Taiyang,the Stomach skin of the Foot-Yangming,and the Foot-Shaoyang Gallbladder Meridian.
