| Objective: At present, there are many kinds of urinary drainage methods used in hypospadias surgery.Three drainage methods were commonly used in our hospital: ordinary urinary catheter method,double-tube urethral stent method and modified double-tube urethral stent method.Based on the analysis and research of the three drainage methods in terms of indwelling time, length of stay in hospital, effective drainage rate and incidence of postoperative complications, etc.,summarize the experience, make an objective evaluation of these three drainage methods, and improve the success of the operation.Rate and reduce related complications. Methods: A total of 147 children with hypospadias who underwent stage Ⅰ urethroplasty during December 2014~December 2019 in our hospital were selected. They were divided into ordinary urinary catheter group, double cannula group and modified according to the different drainage methods. In the double cannula group, the number of samples in each group were 54 cases,55 cases, and 38 cases, and the general clinical data(mean age, hypospadias Duckett classification, operation method)of the three groups were not statistically different(P>0.05);Long-term follow-up after surgery, average 6 months, statistics of common postoperative complications;SPSS 22.0 software was used to perform rank sum test and Chi-square test to compare the number of days of indwelling in the pipeline, the length of hospital stay, the effective drainage rate and related postoperative operations.Differences in complications. Results: There was no statistical difference in the number of days of stent placement for the three urine drainage methods. However, there was a statistical difference in the length of hospital stay(P<0.05),the double cannula group(10.9 days)and the modified double cannula group(10.5 days)were significantly lower than the ordinary urinary catheter group(15.5 days). In terms of effective drainage rate, there was no statistical difference between the three groups(P>0.05);As for postoperative complications of hypospadias,There was no statistical difference in bladder irritation and urethral stricture between the three groups(P>0.05);However,a statistical difference in the incidence of urinary fistula between the general catheter group and the modified double cannula group(P=0.044<0.05).Conclusions:(1)In hypospadias Ⅰ stage urethroplasty, the way of urinary drainage is one of the key factors affecting the success of the operation.(2)Among the three urine drainage methods in this study, the modified double cannula method has obvious advantages, can reduce the incidence of postoperative urinary fistula, and has promotion value.(3)The use of double-tube urethral stent and modified double-tube urethral stent can significantly shorten the hospitalization days of children and reduce medical expenses.(4)Further research still be need to study the individualized factors that affect the complications after hypospadias. |
