Clinical Observation Of RbFGF Combined With Nano-silver Dressing In The Treatment Of Deep Ⅱ Degree Burn Wounds

Objective: This study mainly studied the treatment of rb FGF combined with nano-silver dressing,and clinically observed the difference inclinical efficacy compared with rb FGF alone and nano-silver dressing alone in deep Ⅱ degree burn wounds.In order to find a new treatment plan,to shorten the hospital stay and relieve the suffering of patients,accelerate wound healing,and providerelevant clinical basis for the application and development of rb FGF and nano-silver dressing.Methods:(1)This study selected 72 patients with deep Ⅱ degree burns who were admitted to the Department of Plastic and Burn Wounds Repair Surgery of Affiliated Hospital of Youjiang Medical University for Nationalities from July 2017 to December 2019.All the patients were completely randomized into 3 groups,which were: The rb FGF group,The nano-silver group,and The combination group,24 cases in each group.(2)All the patients were evaluated comprehensively,active comprehensive treatment,and treatment of burn wounds by debridement and disinfection.The rb FGF group was treated with recombinant bovine basic fibroblast growth factor gel for external use.The nano-silver group treated with direct wounds covered with nano-silver dressing.The patients in the combination group were treated with recombinant bovine basic fibroblast growth factor gel for external use,and then covered them with nano-silver dressing.(3)The difference in the time of wound healing,the total response rate of wound healing,the visual analogue score(VAS),the wound healing quality score,the incidence of complications were compared between the three groups.The relevant data were obtained and statistical analysis was performed using SPSS 20.0software.Results:(1)Comparison of general basic information: The patients were divided into the rb FGF group(n=21),the nano-silver group(n=24),and the combination group(n=21).During the observation and treatment of the three groups of patients,a total of 72 patients were lost to 6 patients: 3 were lost to the rb FGF group,and 3 were lost to the combination group.There were no significant differences in gender,age,burn area,and injury factors between three groups.The clinical data were in statistical order(P > 0.05).(2)Comparison of wound healing time:the wound healing time in the three groups was(20.05?1.66)days in the rb FGF group,(19.96 ? 1.90)days in the nano-silver group,and(18.81 ? 1.70)days in the combination group.The wound healing time of the three groups was statistically significant(P < 0.05),and the wound healing time of the combination group was less than the rb FGF group and the nano-silver group(P < 0.05),the wound healing time was not different between the rb FGF group and the nano-silver group(P>0.05).(3)Comparison of the total response rate of wound healing : the total response rate of wound healing in the three groups was28.57% and 47.62% in the rb FGF group(after 14 days of medication、21 days of medication),29.17% and 45.83% in the nano-silver group,s,and 71.43% and 90.48% in the combination group,s.The total response rate of wound healing of the three groups was statistically significant(P < 0.05),and the total response rate of the combination group was higher than the rb FGF group and the nano-silver group(P < 0.017),the total response rate was not different between the rb FGF group and the nano-silver group(P>0.017).(4)Comparison of the visual analogue score(VAS): The median of the visual analogue score(VAS)was 8(7,8)、5(5,6)、3(3,4)and 2(1,2)in the rb FGF group(pre-medication、after 3 days of medication、after 7 days of medication、after 14 days of medication),the median of the visual analogue score(VAS)was 8(7,8)、5(5,6)、2(0.5,3)and1(0.25,2)in the nano-silver group,s,the median of the visual analogue score(VAS)was8(7,8)、5(5,6)、2(1,2)and 1(0.25,1)in the combination group,s.There was no significant difference in the visual analogue score(VAS)between thethree groups(pre-medication、after 3 days of medication)(P > 0.05),and the visual analogue score(VAS)between the three groups(after 7 days of medication 、 after 14 days of medication)was statistically significant(P < 0.05),and the visual analogue score(VAS)of the rb FGF group was less than the nano-silver group and the combination group(P < 0.017),the visual analogue score(VAS)was not different between the combination group and the nano-silver group(P>0.017).(5)Comparison of the wound healing quality score:The median of the wound healing quality score in the three groups was,6(4.5,7)in the rb FGF group,6(4.25,7)in the nano-silver group,and 4(3.5,5)in the combination group.The wound healing quality score of the three groups was statistically significant(P < 0.05),and the wound healing quality score of the combination group was less than the rb FGF group and the nano-silver group(P < 0.017),the wound healing quality score was not different between the rb FGF group and the nano-silver group(P > 0.017).(6)Comparison of the incidence of complications : the incidence of complications in the three groups was57.14% in the rb FGF group,29.17% in the nano-silver group,and 14.29%in the combination group.The incidence of complications of the three groups was statistically significant(P <0.05),and the incidence of complications of the combination group was less than the rb FGF group(P < 0.017),the incidence of complications was not different between the rb FGF group and the nano-silver group(P>0.017),and the incidence of complications was not different between the combination group and the nano-silver group(P>0.017).Conclusion: This study shows that in the treatment of deep Ⅱ degree burn wounds,recombinant bovine basic fibroblast growth factor(rb FGF)combine with nano-silver dressing can shorten wound healing time,effectively promote wound healing,relieve the pain of the patients,improve the quality of early wound healing,reduce the incidence of complications,and be worthy of clinical application.
