ObjectiveTo evaluate brain activity in resting—state of young adults with primary insomnia of fractional amplitude low frequency fluctuation(fALLF).Methods1)Recruited 21 PI patients conforming to the diagnostic criteria of diagnostic and statistical manual of mental disorders,fourth edition(DSM-IV)and 20 healthy controls(HC)with gender,age,level of education matched.All the subjects underwent three-dimensional brain structure and rs-fMRI scan.2)Before the day of the rs-fMRI scan,all participants were evaluated by PSG、Pittsburghsleep quality index scale(PSQI),Brief version of dysfunctional beliefs and attitudes about sleep(DBAS-16),Hamilton depression scale(HAMD),Hamilton anxiety scale(HAMA).3)The preprocessing and the calculation of fALFF for functional images in PI group and HC group were on Matlab 2010a station;the fALFF value between groups were compared respectively by two sample t test with REST software,then the brain areas which are statistically significant were obtained;then Pearson correlation analysis for regions with abnormal fALFF value in PI group with Clinical scale Score were performed respectively using IBM SPSS 19.0 statistical software.Result1)There was no statistical difference between two groups in the age and level(P>0.05);the PSQI,DBAS-16,HAMD,HDMD scores of PI group were significantly than that of the HC group,with difference statistically significant(P<0.05).PI group exists objective sleep disorder。2)Compared with HC group,PI group showed significantly reduced fALFF value in left lateral frontal gyrus(dlPFC)and left precuneuce(P<0.005,Alphasim correction).3)fALFF value in left lateral frontal gyrus positively correlated with DBAS-16(r=-0.465,P=0.035);Conclusion1)PI causes the spontaneous active fALFF value of the dlPFC-L neurons,which may be one of the factors affecting the cognitive and emotional abnormalities of PI patients.2)PI dlPFC-L fALFF values and DBAS-16 scores were positively correlated,speculation may be DLPFC regulating function of the emotional cognitive activities damaged the insomnia disorders caused by negative cognition of sleep.Therapeutic psychotherapy,especially cognitive behavioral therapy,may improve insomnia symptoms by improving the function of the prefrontal cortex.3)PI patients on the left side of the precuneus neurons fALFF value to reduce spontaneous activity,could be PI too pay attention to the increased sensitivity of sleep disorders,sensory stimulation and the cerebral cortex increased emotional reactivity,explore the precuneus and related function connected to the potential role of mechanism to treat problems,sleep and wake and insomnia comorbid depression,anxiety problems is necessary.
