| Purpose:To explore the feasibility and effectiveness of TCM comprehensive efficacy standard for advanced non-small cell lung cancer(NSCLC)under the guidance of the theory of form and spirit.Material and method: By retrospective analysis of case study method,121 patients with advanced non-small cell lung cancer were selected from the Department of oncology,Affiliated Hospital of Liaoning University of traditional Chinese medicine from January 2018 to October 2019.The general data and condition of the patients were comprehensively sorted out,and 4 research indicators including tumor size change,symptom change,KPS score change and mental change before and after treatment were recorded and quantified 19.0software for statistical analysis.The survival prediction model and prognosis index of advanced non-small cell lung cancer were established by Cox multiple factor regression analysis.Based on this,the evaluation method of comprehensive efficacy standard of traditional Chinese medicine was obtained.The results of comprehensive efficacy standard of traditional Chinese medicine of 121 patients were compared with that of improved solid tumor efficacy standard by chi square test and Kaplan Meier survival curve.Results:1.According to Cox multivariate regression analysis,the survival prediction model of advanced NSCLC was h(t,x)= h0(t)exp(0.526 tumor size change + 0.623 symptom change +0.601 KPS score change + 0.730 mental change).The prognostic index(PI)was calculated as pi =0.526 tumor size change + 0.623 symptom change + 0.601 KPS score change + 0.730 mental change.2.The global ?2test for the survival prediction model of advanced non-small cell lung cancer showed that ?2=70.369,?= 4,P<0.001,which indicated that the model was statistically significant and the equation was reasonable.3.The evaluation method of the comprehensive efficacy standard of TCM is as follows:(1)Significant effect: the tumor is effective,the symptoms,KPS and mind are all stable or ?1 effective on the basis of stability;(2)Effective: the tumor is effective,with symptoms,KPS and mind ? 1;or the tumor is stable,with symptoms,KPS and mind ? 2;or the tumor is ineffective,with symptoms,KPS and mind all effective;(3)Stable: the tumor is stable,with symptoms,KPS and mind ? 2 items stable;or the tumor is invalid,with all symptoms,KPS and mind stable;(4)Invalid: the tumor is stable,the symptoms,KPS and mind are all invalid;or the tumor is invalid,the symptoms,KPS and mind ? 2 items are invalid.4.121 patients with advanced non-small cell lung cancer were evaluated according to the comprehensive efficacy standard of traditional Chinese medicine.The effective,effective,stable and ineffective rates were 0,29,53 and 39,respectively.The disease control rate was67.7%.The effective,effective,stable and ineffective rates were 0,19,36 and 66,respectively.The disease control rate was 45.4%,according to Pearson chi square test,?2 =12.264,P< 0.05,indicating that the difference between the two therapeutic standards is statistically significant.5.According to the analysis of the comprehensive curative effect standard of traditional Chinese medicine and the improved curative effect standard of Recist solid tumor combined with the survival period,the survival curve of the comprehensive curative effect standard of traditional Chinese medicine was significantly separated,and the difference was statistically significant(?2=74.251,P=0.000);while the survival curve of the improved therapeutic effect standard of Recist solid tumor crossed each other.Conclusion:1.Under the guidance of the theory of form and spirit,the comprehensive curative effect standard of traditional Chinese medicine can more comprehensively evaluate the overall curative effect of traditional Chinese medicine in the treatment of advanced NSCLC;2.Under the guidance of the theory of relative unity of form and spirit,the comprehensive curative effect standard of traditional Chinese medicine can guide the macro precise treatment of advanced non-small cell lung cancer. |
