Retrospective Analysis Of Treatment Of Coronary Heart Disease Angina Pectoris Complicated With Chronic Heart Failure By Benefiting Temperature And Yang, Removing Phlegm And Distilling Water

Purpose:Through the method of retrospective analysis,the clinical efficacy of benefiting qi and warming yang,resolving phlegm and promoting diuresis method in treating coronary heart disease angina pectoris complicated with chronic heart failure was evaluated,and the clinical experience of the combination of Shenqi Gualou Xiebai Banxia Decoction and Wuling Powder in treating this disease was discussed and summarized.Material and method:From January 2018 to December 2019,outpatients in the affiliated hospital of Liaoning university of traditional Chinese medicine were screened.95 patients with coronary heart disease angina pectoris complicated with chronic heart failure who met the inclusion criteria and exclusion criteria and were treated with modified Shenqi Gualou Xiebai Banxia Decoction and Wuling Powder were selected for retrospective investigation.Clinical data such as symptoms,tongue pulse,electrocardiogram and cardiac ultrasound before and after 4 weeks of treatment were collected respectively.SPSS17.0software was used to statistically analyze the changes of syndrome score,electrocardiogram curative effect,NT-proBNP,ejection fraction(EF value)and cardiac function grading before and after treatment in order to evaluate the clinical curative effect of Shenqi Gualou Xiebai Banxia Decoction combined with Wuling Powder on coronary heart disease angina pectoris complicated with chronic heart failure.Results:1.Traditional Chinese medicine syndrome: After taking the medicine for 4 weeks,the symptoms of chest pain,chest tightness,shortness of breath,fatigue,aversion to cold and limb cold,edema,oliguria,abdominal distension,palpitation,cough,expectoration,asthma,tongue,tongue coating,pulse were improved(P<0.05),and the total effective rate of TCM syndrome effect of 95 patients was 88.42%.2.Electrocardiogram curative effect: After treatment,10 cases(10.53%)showed marked effect on electrocardiogram curative effect.65 cases were effective,accounting for 68.42%;20cases were invalid,accounting for 21.05%;The total effective rate was 78.95%.3.NT-proBNP level: The average value of NT-proBNP before treatment was(2242.90 ±986.102)ng/L,and the average value of NT-proBNP after treatment was(1783.65 ±854.441)ng/L.The NT-proBNP level of patients was improved compared with that before treatment((P<0.05)).4.Ejection fraction(EF value): The average ejection fraction before treatment was(45.55±7.298)% and the average ejection fraction after treatment was(49.35±7.240)%.The ejection fraction of patients was improved compared with that before treatment(P<0.05).5.Classification of cardiac function: After treatment,13 patients(13.69%)showed marked improvement in cardiac function grading,66 cases were effective,accounting for 69.47%;16 cases were invalid,accounting for 16.84%;The total effective rate was 83.16%.Conclusion:1.Shenqi Gualou Xiebai Banxia Decoction and Wuling Powder can obviously improve the clinical symptoms and signs of patients with coronary heart disease angina pectoris complicated with chronic heart failure(deficiency of qi and yang,phlegm coagulation and drinking stop syndrome).2.Treating coronary heart disease angina pectoris complicated with chronic heart failure by benefiting qi and warming yang,resolving phlegm and promoting diuresis can improve ECG,NT-proBNP,ejection fraction and cardiac function grading of patients,and has certain clinical curative effect.
