Professor Zhang Lan's Research On The Characteristics Of Medications For Differentiation And Treatment Of Hashimoto's Thyroiditis

Purpose : The prescription of Professor Zhang Lan for Hashimoto’s thyroiditis was systematically analyzed by data mining technology,thereby concreting Professor Zhang Lan’s medication characteristics and academic ideas for the treatment of Hashimoto’s thyroiditis.Material and method:Collecting and sorting out 147 patients,345 prescriptions of Hashimoto’s thyroiditis diagnosed and treated by Professor Zhang Lan from September 2017 to December 2019,compiling and summarizing the prescriptions of patients,using WPS OFFICE 2019 software to establish a database,using SPSS Statistics 22.0 and SPSS Modeler18.0 software carries out frequency analysis,cluster analysis,factor analysis,association analysis,etc.Results:1.Frequency analysis results:Among 345 prescriptions,the total frequency of medication was6068 times,168 herbs were used,among them,there were 44 traditional Chinese medicines except below the average,which are Professor Zhang Lan’s high-frequency traditional Chinese medicines for the treatment of Hashimoto’s thyroiditis,with a statistical frequency of5084 times and a cumulative frequency of 83.78%.The use frequency of plain drugs is the highest in the drug properties of traditional Chinese medicine,followed by the warm and slightly cold medicine;Sweet drugs are the most used in traditional Chinese medicine,followed by the bitter and pungent drugs,and then the acid,astringent and light taste;The most frequently used traditional Chinese medicine meridians are spleen meridian,liver meridian,lung meridian,kidney meridian and heart meridian.For the category part of traditional Chinese medicine,large categories of drugs are mostly tonifying deficiency drugs,regulating qi drugs,astringent drugs,heat-clearing drugs,diuretic and dampness-permeating drugs,and tranquilizing drugs,subcategories are mostly qi-tonifying drugs,regulating qi drugs,blood-tonifying drugs,essence-strengthening,urination-reducing and bandage-stopping drugs,yin-tonifying drugs,diuretic and edema-eliminating drugs,mind-nourishing and tranquilizing drugs,heat-clearing and blood-cooling drugs.2.Cluster analysis results: Cluster analysis was used to analysis the high-frequency traditional Chinese medicines,and the results showed that six pairs of commonly used drugs,four groups of commonly used three-drug group and four groups of commonly used multi-drug group.3.Factor analysis results: Factor analysis was used to analysis the high-frequency traditional Chinese medicines.After analysis,10 common factors were obtained which can reflect Professor Zhang Lan’s prescription medication academic ideas of treating Hashimoto’s thyroiditis from the angle of soothing liver and invigorating spleen,regulating qi,blood and body fluid from different angles and levels.4.Association analysis results: The results of two-linked association analysis of the high-frequency traditional Chinese medicines were arranged according to the degree of support plus confidence,and the results obtained were consistent with Professor Zhang Lan’s medication characteristics.Conclusion:1.Professor Zhang Lan’s main drug categorie for the treatment of Hashimoto’s thyroiditis is tonifying deficiency drugs;the main drug properties are calm,warm and slightly cold;the main drug taste is sweet,bitter and pungent flavors.2.Professor Zhang Lan mostly used the drug pairs and drug groups of tonifying qi and invigorating spleen,soothing liver and regulating qi,drying dampness and resolving phlegm,activating blood circulation and removing stasis,clearing the liver and purging fire,nourishing yin and invigorating body fluid,nourishing blood and tranquilizing mind,softening firm and dispersing knots,nourishing liver and kidney in the treatment of Hashimoto’s thyroiditis.3.Professor Zhang Lan divided the treatment of Hashimoto’s thyroiditis into three stages:Hashimoto’s thyroiditis in subclinical stage belongs to the type of liver depression and spleen deficiency,which should be treated properly to soothe the liver and strengthen the spleen;Hashimoto’s thyroiditis in hyperthyroidism stage belongs to the type of liver fire hyperactivity or heart liver yin deficiency,which should be treated properly to clear the liver and purge fire or nourish yin and clear heat;Hashimoto’s thyroiditis in hypothyroidism stage belongs to the type of spleen and kidney yang deficiency,which should be treated properly to warm and tonify the spleen and kidney. 4.The results of data mining are consistent with Professor Zhang Lan’s theory and clinical experience in the treatment of Hashimoto’s thyroiditis.
