Research Background:Non-alcoholic fatty liver disease refers to the elimination of alcohol and other clear liver damage factors.The lesions are mainly in the hepatic lobule,a clinicopathological syndrome characterized by diffuse hepatocellular bullous steatosis and fat storage.Non-alcoholic steatohepatitis is a key period of non-alcoholic steatohepatitis.It develops from non-alcoholic hepatic steatosis,and it can also change to cirrhosis and liver cancer.In the early stage of non-alcoholic steatohepatitis,it may be asymptomatic.With the development of the disease,symptoms and signs such as fatigue,right flank pain,mouth pain,hepatomegaly,loose stools or fullness may appear.The pathogenesis of non-alcoholic steatohepatitis is not yet clear.More and more scholars recognize the "multiple shock" theory,which includes insulin resistance,oxidative stress,inflammatory response,lipid peroxidation,intestinal flora,adipokines,genetic polymorphism and epigenetics.For the treatment of non-alcoholic steatohepatitis,there are currently no specific drugs.Commonly used drugs include insulin sensitizers,liver protection and enzyme-lowering drugs,lipid-lowering drugs,and drugs for regulating intestinal flora,and with a reasonable diet and appropriate increase in exercise to self-regulate.Chinese medicine believes that non-alcoholic steatohepatitis is due to liver disorders,spleen and kidney deficiency,phlegm,dampness,turbidity,blood stasis,heat and other pathological factors accumulated within.On this basis,instructor Professor Xie Chun’e believes that its treatment should mainly eliminate phlegm and dampness,clear heat and remove blood stasis,and created Yinchen Linggui prescription.Clinically using this method to add or subtract treatment for non-alcoholic steatohepatitis patients often achieves good results.Previous clinical studies have also shown that Yinchen Linggui prescription can effectively treat the clinical symptoms of non-alcoholic steatohepatitis patients,reduce CAP value,improve liver imaging changes,improve liver function,and correct lipid metabolism disorders.In order to further clarify the intervention effect and mechanism of Yinchen Linggui prescription on non-alcoholic steatohepatitis,this experiment was done.Purposes:To explore the effect of Yinchen Linggui prescription on lipid metabolism,insulin resistance and oxidative stress in non-alcoholic steatohepatitis ratsMethods:After 60 SPF male SD rats were fed adaptively for 7 days,they were randomly divided into normal group(10)and model group(50).The normal group was fed with normal feed and the model was fed with high-fat feed for 8 weeks.Randomly take 2 rats in the model,and determine the success of the model,then randomly divide the remaining 48 rats in the model into the model group(8),Yinchen Linggui prescription high-dose group(10),Yinchen Linggui prescription medium-dose group(10),Yinchen Linggui prescription low-dose group(10),and polyene phosphatidylcholine group(10);corresponding drugs were given for 4 weeks.The body weight and wet weight of the liver were measured to calculate the liver index,and the fasting serum GLU and INS levels were measured to calculate HOMA-IR.Serum ALT,AST,T-CHO,TG and FFA levels were detected.The levels of MDA,SOD and GSH-PX in liver tissue of rats were measured.LEP and APN levels in serum and liver tissues of rats were measured,and the liver tissues of rats were taken for hematoxylin-eosin staining to observe the morphological changes of liver tissues and calculate the NAS scores of liver tissuesResults:①Compared with the normal group,the body weight,liver index,HOMA-IR and serum ALT,AST,T-CHO,TG,FFA,MDA level of liver tissue and NAS score of liver tissue in the model group were significantly increased(P<0.01 or P<0.05);SOD and GSH-PX levels in liver tissue were significantly reduced(P<0.01);LEP levels in serum and liver tissue homogenates were significantly increased(P<0.01),and APN levels were significantly reduced(P<0.01)·②Compared with the model group,the body weight,liver index,HOMA-IR,serum ALT,AST,T-CHO,TG,FAA,MDA level of liver tissue and NAS score of liver tissue in the high-dose group of Yinchen Linggui prescription were significantly reduced(P<0.01 or P<0.05),the levels of SOD and GSH-PX in liver tissue increased significantly(P<0.01),and the levels of APN in serum and liver tissue of rats increased significantly(P<0.01);The liver index,HOMA-IR and serum ALT,AST,T-CHO,TG,MDA level of liver tissue and NAS score of liver tissue were significantly decreased in the middle-dose group of Yinchen Linggui prescription(P<0.01 or P<0.05),liver tissue GSH-PX levels increased significantly(P<0.01),serum and liver APN levels increased significantly(P<0.01);The liver index,HOMA-IR and serum AST,T-CHO levels and liver tissue NAS score of rats in the low-dose group of Yinchen Linggui prescription were significantly decreased(P<0.01 or P<0.05),and serum APN and liver tissue GSH-PX levels were significant Increase(P<0.01 or P<0.05);The body mass,liver index,HOMA-IR and serum ALT,AST,T-CHO,TG,FAA,MDA level of liver tissue and NAS score of liver tissue were significantly decreased in the polyene phosphatidylcholine group(P<0.01 or P<0.05),SOD and GSH-PX levels in liver tissue were significantly increased(P<0.01 or P<0.05),APN levels in serum and liver tissue were significantly increased(P<0.01),LEP levels in serum and liver tissue were significantly reduced(P<0.01 or P<0.05).Conclusions1.Feeding rats with high-fat diet for 8 weeks can replicate the NASH model2.Yinchen Linggui prescription can reduce the body weight and liver wet weight of NASH rats.It can prevent and cure NASH by reducing liver fat accumulation and inhibiting obesity.3.Yinchen Linggui prescription can increase the expression of APN,prevent lipid deposition in the body and reduce the damage of insulin resistance to the body.4.Yinchen Linggui prescription can adjust the dynamic balance between SOD,GSH-PX and MDA by reducing the MDA level and increasing the level of SOD and GSH-PX,thereby improving the antioxidant capacity of NASH rats and preventing and treating NASH.
