| BackgroundPost-stroke emotion disorder is a common complication of stroke patients,which has a serious impact on the physical and mental health of stroke patients,patients of stroke is reduced the enthusiasm of rehabilitation exercise,and it increases the mortality rate and disability rate of stroke,and brings psychological and economic burden to families.In clinical practice,early detection,early prevention and early treatment of post-stroke emotion disorders are extremely important.For post-stroke emotion disorders,there are the drug therapies and the non-drug therapy currently available for treatment.Drug therapies are effective,but its side effects cannot be effectively avoided.There are many kinds of non-drug therapies with good safety,but their efficacy is still uncertain,and there is insufficient clinical evidence to confirm their therapeutic effect.ObjectiveTo observe the preventive effect of five-element music on emotion disorders in stroke patients in convalescence,and to evaluate the effect of five-element music on emotion,nerve function,limb motor function and daily activity ability of stroke patients in convalescenceMethodUsing randomized controlled clinical trial,72 patients with stroke in convalescence were randomly divided into five-element music therapy group and routine rehabilitation treatment group.One group was given conventional rehabilitation combined with five elements of music,the other without five-element music,both for 8 weeks.In 4 weeks,8 weeks,we evaluated the two groups of the patient’s emotional state(HAMA and HAMD),neurological conditions(NIHSS score),daily activity ability(Barthel index),limb motor function(arms and lower limbs Brunnstrom,a simplified Fulg-meyer functional assessment)and TCM symptoms scale,compared two groups of each time point score is statistically significant.Result1.Emotional condition:After 8 weeks of treatment,there was no significant difference in the incidence of depression and anxiety between the control group and the experimental group(P<0.05).After 8 weeks of treatment,compared with the control group,the HAMD score distribution of the experimental group was statistically different(P<0.05),the HAMA score distribution was not statistically significant(P>0.05),Significantly more patients without depression than in the control group.In experimental group,the Hamilton depression scale score in the experimental group was significantly decreased,with statistically significant difference(P<0.05).The score of Hamel anxiety scale was significantly decreased with statistically significant difference(P<0.05).2.Neurological function:after 8 weeks of treatment,NIHSS score of the two groups significantly decreased(P<0.05).After 8 weeks of treatment,there was no significant decrease in NIHSS score of the experimental group compared with the control group(P<0.05).3.Daily activity ability:after 8 weeks of treatment,Barthel index score of the two groups was significantly increased(P<0.05),and after 8 weeks of treatment,compared with the control group,Barthel index score of therapy group was not significantly increased(P<0.05).4.Limb motor function:after 8 weeks of treatment,the Brunnstrom of upper and lower limbs and Fulg-meyer scores were significantly improved(P<0.05).After 8 weeks of treatment,there was no significant difference in Brunnstrom score and Fulg-meyer score between the therapy group and the control group(P<0.05)5.TCM symptoms:Comparison of two groups,after 8 weeks of treatment,the symptoms of "sadness to cry","sigh frequently","shortness of breath","spontaneous sweating","chest tightness" and "inappetence" were significantly improved in the therapy group compared with the control group,and there were significant differences between the therapy group and the control group.The therapy group was superior to the control group(P<0.05).Control group didn’t improve before and after treatment(P<0.05).In the therapy group before and after treatment,the symptoms of "easy to sulking","sigh frequently","fatigue","abdominal distension" were significantly improved after treatment than before,and there were statistical differences(P<0.05).Conclusion1.After 8 weeks of treatment,Five-element music could not effectively prevent the occurrence of post-stroke emotion disorders.But Five-element music can improve the potential anxiety and depression of stroke patients.2.After 8 weeks of treatment,Five-element music did not significantly improve the neurological function,daily activity ability and body movement function of patients without emotion disorder during the convalescence stage3.In the 8-week treatment cycle,Five-elements music may have a significant improvement effect on patients who are stagnation of liver qi with deficiency of spleen syndrome. |
