Objective: Patellofemoral Pain Syndrome(PFPS)is a common and frequently-occurring disease in clinic,which seriously affects the quality of life and motor ability of patients.Traditional passive treatment is ineffective and prone to recurrence.Studies have shown that quadriceps femoris exercise intervention is effective.By comparing the improvement of different clinical symptoms of PFPS patients with different quadriceps femoris exercise intervention methods,this study explored the relationship between flexibility and strength deficiency of quadriceps femoris and the occurrence and development of different clinical symptoms of PFPS patients,and provided theoretical basis for the prevention and treatment of PFPS by exercise intervention;and further through observing the improvement effect of different intervention schemes on different clinical symptoms.To explore suitable exercise intervention schemes for different clinical symptoms,so as to provide clinical basis for exercise intervention in the treatment of PFPS.In order to provide the basis for clinicians to choose the best exercise intervention program,improve the pertinence of clinical exercise intervention program,and reduce the economic and mental burden of patients.Methods: In this study,63 PFPS patients who met the exclusion criteria of this study were enrolled in a single-center,randomized,single-blind controlled trial from May 2018 to September 2018.They were randomly assigned to three treatment groups: quadriceps femoris strength training group,quadriceps femoris stretching training group,and control group.They were given corresponding exercise intervention five times a week for 6 weeks.Demographic data were collected and Kujala Patellofemoral Score(KPS)and Visual Analogue Scale/Score(VAS)were tested before and after the intervention,as well as knee extension,knee flexion and quadriceps flexibility.The changes of clinical symptom scores,quadriceps femoris strength and flexibility before and after intervention were compared.Results: Sixty-three of the 63 subjects completed the experiment without early withdrawal from the experiment and adverse events.There was no significant difference in each index before the intervention among the three groups(P > 0.05).After intervention,there were significant differences in the main outcome indicators of KPS(p=0.02),strength training group and control group(p=0.01),stretch training group and control group(p=0.55),and strength training group and stretch training group(p=0.04).Secondary outcome indicators daily VAS scores after intervention had significant difference among the three groups(p = 0.01),strength group andcontrol group had significant difference(p = 0.00),stretch group and control group had no significant difference(p = 0.07),strength group and stretch group had no significant difference(p = 0.20).There was no significant difference among the three groups in the VAS score after knee flexion and sedentary exercise(p=0.08).There were significant differences among the three groups(p=0.01),strength group and control group(p=0.00),stretch group and control group(p=0.18),strength group and stretch group(p=0.09).There were significant differences among the three groups(p= 0.00),strength group and control group(p = 0.00),stretching group and control group(p = 0.18),strength group and stretching group(p = 0.04).After the intervention of squatting VAS score,there were significant differences among the three groups(p = 0.00),strength group and control group(p = 0.00),stretching group and control group(p = 0.00),strength group and stretching group had no significant difference(p = 0.17).There was no significant difference in the VAS score of jogging between the three groups(p=0.05),the strength group and the control group(p=0.01),the stretching group and the control group(p=0.27),and the strength group and the stretching group had no significant difference(p=0.16).There was no significant difference between the three groups after knee extension and knee flexion strength intervention(P > 0.05).There was significant difference among the three groups after quadriceps flexibility intervention(p = 0.00).There was no significant difference between the strength group and the control group(p = 0.08).There was significant difference between the stretching group and the control group(p = 0.02).There was significant difference between the strength group and the stretching group(p = 0.00).Conclusion: For comprehensive improvement of knee joint function and exercise condition of PFPS patients,quadriceps femoris strength training is effective.For relieving PFPS patients' pain in daily life,up stairs,down stairs,and jogging,quadriceps femoris strength training is effective too.For relieving PFPS patients' pain in squatting,both quadriceps femoris strength training and stretching training are effective.For relieving PFPS patients' pain in sitting with flexion of knees,neither quadriceps femoris strength training nor stretching training are effective.
