Objective Auditory brainstem response is a common neuro-electrophysiological test technique for detecting hearing levels and auditory brainstem function,acoustically evoked short latency negative potential is a vestibular neurogenic potential found in auditory brainstem response.Generally,ASNR could not be recognized due to the overlap of ABR.On the contrary,the waveform of ABR was also mixed with ASNR potential.Separated two potentials by white noise masking to obtain the real detection value.In the early stage,the research group has extracted ASNR from normal volunteers through the white noise masking of ABR.In this study,an optimized strategy was selected to obtain a higher induction rate of ASNR,collect the parameter of normal population such as threshold,incubation period,amplitude under the optimized ASNR.By reducing the cochlear response with white noise to verify the conjecture of ABR-ASNR = "pure" ABR.Compare the similarities and differences between normal ABR and "pure" ABR,explore the necessity of purification and the condition of purification exemption conditions and evaluate the clinical value both ASNR and "pure" ABR.Methods 20 normal hearing subjects(40 ears)were employed.Air conduction click-ABR and white noise ipsilateral masking ASNR were tested before and after.Recorded the parameter potential latency,occurrence rate and relationships twice or more each stimulation intensity.The group comparison of different masking methods,signal-noise-difference,stimulation rate and stimulation mode were conducted.Determine the most optimized strategy and masking threshold with statistical analysis to acquire“pure”ABR and discuss the clinical and value of ASNR and “pure”ABR.Results The extraction rate of ABR was 100%,and 27 ears of ipsilateral-masking evoked to ASNR(67.5%).Extraction rate high,the average amplitude of ASNR was 0.184±0.062(uv)and the average latency was3.29±0.30(ms)at 95 d B n HL.The stimulus intensity range of ASNR was85-100 d B n HL,Earmuffs masking is superior to insertion masking and the optimal signal-to-noise difference for WN masking was 30 d B n HL.The purified ABR was statistically different from the original ABR on latency but on attitude for III wave.Conclusion We successfully evoked ASNR with Ipsilateral white noise masking in normal people through the self-made model.Air conduction Ipsilateral masking is a feasible ASNR evoking strategy,and the collection of normal ASNR data can be used to guide the establishment of ASNR diagnosis technology.The superposition between ASNR and ABR was verified.Purification operation can reflect the parameter of ABR III wave accurately and has the potential to be another monitor indicator with diagnostic significant.
