Knee osteoarthritis(KOA)is the most common progressive joint disease involving cartilage and surrounding tissues.It is usually characterized by joint inflammation and repair bone response.It is the main cause of knee joint pain,motor dysfunction,and even disability.the reason.Most patients with osteoarthritis have mild initial symptoms,but if they do not receive relevant treatments,their condition will gradually get worse,with knee joint stiffness and deformation,severe pain when going up and down stairs,and squatting.The disease not only seriously affects the health and quality of life of patients,but also brings a huge economic burden to individuals and the healthcare system.Objective This subject is a randomized controlled study of clinical subjects.Three different treatment methods are used in clinical practice: leukocyte-rich platelet-rich plasma(LR-PRP)injection,leukocyte-deficient platelet-rich plasma(LP-PRP)injection and exercise therapy,and observe the three treatments Methods The early clinical curative effect of knee osteoarthritis,in order to provide the best treatment method for clinical treatment of knee osteoarthritis.Methods 51 patients with clinically diagnosed knee osteoarthritis in the Department of Rehabilitation Medicine of the Chinese People's Liberation Army General Hospital were randomly divided into exercise therapy group,LR-PRP group and LP-PRP group.The diagnostic criteria refer to the diagnostic criteria for osteoarthritis(OA)formulated by the International Association for the Study of Osteoarthritis in 2015 to confirm the diagnosis of the disease.The exercise therapy group was given lower limb muscle training intervention,5 times a week,for a total of 4 weeks;the LR-PRP group and LP-PRP group were injected with PRP with different concentrations of white blood cells,once a week,for a total of 3 times.The treatment effects of each group were evaluated before treatment,at the first month and at the third month.Curative effect observation index items are: visual analog pain score scale(VAS),osteoarthritis index(WOMAC)score,knee joint function(IKDC)score,knee joint flexor and extensor isokinetic test peak torque value(PT),total work(TW).Results Comparison before and after treatment:(1)Comparison within the group: All three treatment methods can reduce the VAS score of KOA patients(all P<0.05).Comparison between groups: The improvement of VAS in the LP-PRP and LR-PRP groups was better than that of the exercise therapy group,the difference was statistically significant(all P<0.05),and there was no statistical difference between the LP-PRP group and the LR-PRP group Significance(P>0.05),suggesting that the clinical effect of intra-articular injection of PRP to relieve pain is better than exercise therapy.(2)Comparison within the group: All three treatment methods can improve knee joint function IKDC(all P < 0.05).Comparison between groups: the improvement of knee joint function in the LP-PRP group was significantly better than that in the LR-PRP and exercise therapy groups(P<0.05),and the improvement in knee function in the exercise therapy group was more significant than that in the LR-PRP group,and the difference was statistically significant.(P<0.05).(3)Comparison within groups: The WOMAC scores of knee arthritis index in the three groups were significantly reduced after treatment(all P<0.05).Comparison between WOMAC groups: The decrease in knee index in the LP-PRP group was significantly better than that in the LR-PRP and exercise therapy groups(P<0.05),and the decrease in the WOMAC score of the knee index in the LR-PRP group was more significant than that in the exercise therapy group.There were differences Statistical significance(P<0.05),suggesting that the clinical effect of intra-articular injection of LP-PRP in improving knee arthritis index is better than that of LR-PRP and exercise therapy group.(4)Comparison of the peak torque(PT value)and total work(TW value)of the three groups of isokinetic tests after treatment: the three treatment methods have improved the peak torque and total work of the flexor and extensor muscle groups,with differences There is statistical significance(all P<0.05).Among them,for the improvement of the peak torque of the flexors and extensors,the improvement of the exercise therapy group and the LP-PRP group was better than that of the LR-PRP group(P<0.05),and there was no statistical significance between the exercise therapy group and the LP-PRP group(P>0.05).For the total work of the flexors and extensors,the improvement degree of the LP-PRP group and the exercise therapy group was better than that of the LR-PRP group(P<0.05),and the exercise therapy group was compared with the LP-PRP group,and the LP-PRP group improved the most obvious.Conclusion LP-PRP injection therapy is better than LR-PRP in improving knee joint function,flexor and extensor peak torque,and total work;LR-PRP injection therapy is better than exercise therapy in relieving pain;exercise therapy is better in improving knee joint function,flexor and extensor muscles Peak torque and total work are better than LR-PRP group;LP-PRP injection therapy is better than exercise therapy in relieving pain and improving knee joint function.
