Piweisan is a compound made from Radix Codonopsis,stir-fried Atractylodes,Poria and other materials,the main functions are to replenish the spleen and the stomach,regulate qi and dissipate accumulation,promote body fluid and stop adverse reactions,the main treatment diseases are chronic gastric and esophageal diseases caused by insufficient secretion of gastric acid and digestive enzymes.Therefore,this article takes the traditional Chinese medicine prescription of piweisan for the treatment of Chronic Atrophic Gastritis(CAG)as the research object Aspergillus oryzae NBM-3 for food and drug use was taken as a strain,the amount of three auxiliary materials used in fermentation,including Triticum aestivum,Artemisia carvifolia and wheat flour wasoptimized,and then the fermentation conditions were optimized at the level of the triangular flask,the fermented traditional Chinese medicine compound piweisan was toxicologically safety and then further used in clinical research.The main conclusionsare as follows:(1)Optimization of fermentation process of piweisanThe solid-state fermentation medium of piweisan was optimized by the simplex centroid method,and the final formula obtained was 9 g Triticum aestivum,9 g Artemisia carvifolia,and 96 g piweisan.The reduction rate of decoction extract(solid matter)is 73.23%.On this basis,the optimal fermentation conditions for the triangular flask level using the Box-Benhnken method are the ratio of material to water is 1:1.40,the inoculation amount is 2.9×10~6a/g,the fermentation time is 4.50 d,and the reduction rate of the total peak area(the sum of the peak areas of each component)in the HPLC chart of piweisan is 95.94%.Amplify the above experimental results were enlarged to a 30 L bioreactor for solid-state fermentation,after research,the optimal ratio of material to water was 1:0.6.(2)Changes in main components of the traditional Chinese medicine piweisan compound after fermentationThe post-fermented samples were analyzed by liquid chromatography and molecular gel chromatography.From the analysis of the liquid chromatogram,it can be seen that compared with the unfermented samples,the peak area of the fermented samples(peak time:9.00 min,25.40 min,26.00 min)was greatly reduced.From the analysis of the gel chromatogram,the proportion of proteins with a molecular weight of>670 k Da in unfermented samples is 45.61%,while the proportion of the fermented samples is only 5.99%,indicating that the fermentation has transformed the macromolecular active substances into small molecules.Substances improve the curative effect,and certain toxic macromolecular substances are also degraded,reducing toxic and side effects,and playing a better role in increasing efficiency and reducing toxicity.(3)Toxicology evaluation of the traditional Chinese medicine piweisan compoundThrough acute and chronic toxicological experiments,it can be known that this formula is non-toxic,while the unfermented formula causes drug-induced damage to the liver.The fermented formula reduces this toxic side effect,indicating that it has a certain degree of safety and qualification for long-term use.(4)Clinical study of the traditional Chinese medicine piweisan compoundThrough clinical observation data analysis of 229 patients with spleen and stomach weakness and gastric qi stagnation type chronic gastritis,it was found that the total effective rate(97.41%)of the observation group taking fermented spleen and stomach powder was higher than that of the control group(84.07%)taking unfermented spleen and stomach powder,it shows that the clinical treatment effect of fermented spleen and stomach powder is better,safe and reliable.
