Objective:To evaluate the feasibility of lobectomy with video-assisted thoracoscopic surgery(VATS)in the management of intralobar pulmonary sequestration and summarize our experience. Methods:From March 2000 to June 2015,thirty six patients with intralobar pulmonary sequestration underwent lobectomy with VATS or with thoracotomy in our department. These patients were divided into VATS group and thoracotomy group.Clinical data, such as operative time, blood loss, the amount of chest tube drainage, postoperative hospital stay and complications were analyzed. Results:The operations included 14 lobectomies of right lower lobe,21 of left lower lobe and 1 of left lower lobe plus lingular segmentectomy. There were 23 patients in the thoracotomy group and 13 in the VATS group in which no one was transferred to thoracotomy. In two groups, the differences of the blood loss(P=0.007),the chest tube duration(P=0.039)and the length of postoperative hospital stay(P=0.004)were statistically significant. The differences in operative time(P=0.202),amount of chest tube drainage(P=0.591)and postoperative complications(P=0.682)were not significant statistically. There was no mortality and all patients recovered uneventfully.Thirty one patients were followed up after surgery from 6 months to 15 years and all recovered well.Five cases were lost to follow-up.Conclusion:VATS lobectomy is safe and effective in the management of intralobar pulmonary sequestration.
