Research objective:To compare the curative effects of craniotomy and transfrontal puncture and drainage in patients with hypertensive basal ganglia hemorrhage,and to explore the individualized treatment of hypertensive cerebral hemorrhage.Methods:191 patients with hypertensive basal ganglia hemorrhage were selected from the Third Affiliated Hospital of Suzhou University from June 2015 to May 2018.Among them,108 patients(craniotomy group)underwent craniotomy and 83 patients(puncture group)underwent rated puncture and drainage.According to the latest World Health Organization(WHO)score on age,the patients were divided into youth group,middle-aged group and elderly group.According to GCS score,the patients were divided into severe and non-severe groups.The operation time,postoperative complications,30-day mortality and daily life of three months after operation were compared.Activity ability(ADL)score.Research results:1.There was no significant difference in age,sex,timing of operation,volume of hematoma and severity of the disease among different operation groups.2.Comparison of the two types of operation in different age groups:_Youth group:puncture operation less time than craniotomy,less postoperative complications,the above differences were statistically significant,there was no significant difference in mortality,prognosis between the two types of operation;_Middle-aged group,elderly group:puncture than craniotomy operation time,postoperative complications,postoperative complications.The postoperative mortality rate was lower,and the prognosis was better.3.Comparison of the curative effect of two kinds of operation:_Non-severe group(GCS>8 points):Puncture group than craniotomy group with less operation time,less postoperative complications,the above difference was statistically significant,there was no significant difference between the two methods of prognosis;_Severe group(GCS<8 points):puncture group than craniotomy group with surgery time,postoperative and The morbidity(pneumonectomy rate,severe pneumonia,digestive tract reaction,incision reaction),postoperative mortality were lower and the prognosis was better,the difference was statistically significant(P<0.05).4.Comparison of curative effect of two kinds of operation for cerebral hernia patients:the operation time of puncture group is less than that of craniotomy group,the postoperative complications are less,and the prognosis is better.The difference is statistically significant.Conclusion:Puncture is less time-consuming,less trauma,less postoperative complications,the overall effect is better than craniotomy,middle-aged and elderly patients with hypertensive basal ganglia hemorrhage can be the preferred operation.
