Objective:Based on the theory of "attaching equal importance to both bones and tendons",the operation concept and clinical effect of TLIF combined with contralateral percutaneous screw fixation and PLIF in the treatment of lumbar degenerative diseases were analyzed,which provided valuable clinical ideas for the treatment of lumbar degenerative diseases.Methods:In this study,60 patients were randomly selected from September 2016 to September 2018 in the same hospital affiliated to Shandong Uni vers i ty of Traditional Chinese Medicine for posterior lumbar interbody fusion.They were divided into treatment group(modified TLIF combined with contralateral percutaneous screw placement group)and control group(PLIF group),30 cases in each group.The average operation time,intraoperative bleeding volume,knife-edge length,postoperative drainage and 6-month bone graft fusion rate were recorded and compared between the two groups.Visual Analogue Scale(VAS),Oswestry disability index(ODI)and functional score were used to evaluate the effect of the operation.SPSS Statistics 20.0 statistical software was used to analyze the data.Results:1.In terms of operation time,the improved TLIF group was shorter than the PLIF group,and the difference between the two groups was statistically significant(P<0.05).2.In terms of intraoperative bleeding volume,PLIF group was higher than modified TLIF group,and the difference was statistically significant(P<0.05).3.The length of knife edge in PLIF group was longer than that in TLIF group(P<0.05).4.In terms of postoperative drainage,the PLIF group was higher than the modified TLIF group,and the difference was statistically significant(P<0.05).5.In terms of VAS and ODI scores,both groups improved significantly after operation.Conclusion:In terms of therapeutic effect,both modified TLIF and PLIF can achieve satisfactory therapeutic effect,but the modified TLIF group is superior to PLIF group in terms of surgical trauma,which is more in line with the theory of "laying equal stress on bones and muscles".This surgical method is worth popularizing and applying in clinic.
