Purpose:To observe the dynamic changes and clinical efficacy of diabetic macular edema(DME)in patients with diabetic macular edema(DME)before and after intrabulicular injection of anti-VEGF drugs.Methods:Twenty patients(35 eyes)with diabetic macular edema were enrolled prospectively from December 2018 to October 2019 in the Department of Ophthalmology of the 90th Hospital of the PL A Joint Service.Color Doppler ultrasound was used to detect the blood flow of the central retinal artery(CRA),ophthalmic artery(OA),and posterior ciliary artery(PCA)in the patients before and one week after the injection of anti-VEGF drugs before and after intravitreal injection Parameters,including peak systolic velocity(PSV),end-diastolic blood flow velocity(EDV),and resistance index(RI),to compare and analyze the hemodynamic changes of the eyeball before and after treatment;at the same time,compare the macular area before and after treatment Changes in retinal thickness,best corrected vision,blood pressure,heart rate,and intraocular pressure.Results:The average age of 20(35 eyes)DME patients was(60.15 ± 12.35)years old,including 11 males(18 eyes)and 9 females(17 eyes).The OA,CRA,PCA of the eyes were injected on the first day before surgery.The comparison of blood flow indexes PSV,EDV,and RI with 1 week after operation showed that PSV of OA,CRA,and PCA decreased significantly(P=0.000,P=0.015,P=0.011,respectively),P<0.05,there were Statistical significance;EDV of OA and PCA decreased significantly(P=0.009,P=0.026,respectively),p<0.05,was statistically significant;RI of OA,CRA,PCA was not statistically significant(P>0.05);The retinal thickness of the macular region 1 day before and 1 week after injection were respectively 392.45±5.24 um、362.16±6.75um,the difference was statistically significant(P=0.012);the best corrected visual acuity(BCVA)of 1 day before injection was significantly improved compared with 1 week after injection,and the difference was significant.There was statistical significance(P=0.032);blood pressure,heart rate,and intraocular pressure were not statistically significant one day before and one week after injection(P>0.05).Conclusion:1.Intravitreal injection of anti-VEGF drugs has a certain safety and effectiveness;2.Intravitreal injection of anti-VEGF drugs can reduce ocular blood flow to a certain extent,aggravate retina and optic nerve ischemia,clinically For these patients,it is necessary to further strengthen the treatment to improve eye circulation and protect the optic nerve;3.After anti-VEGF drugs,the effect of anti-VEGF drugs can reduce the effect of ocular blood flow when evaluating the prognosis of the disease;VEGF drugs can significantly improve the degree of macular retinal edema and the best corrected visual acuity in the short term.
