Objective: To study the effect of acute normovolemic hemodilution(ANH)on the perioperative blood protection of artificial hip arthroplasty(RHA)on the basis of using Isa.Methods: from 2015 to 2019,30 cases of hip arthroplasty in orthopaedic department of Guangxi Zhuang Autonomous Region People’s hospital were analyzed retrospectively.30 patients were divided into two groups.In the experimental group,acute isovolumic hemodilution combined with recycled autotransfusion technology was used for blood protection in the perioperative period;in the control group,acute isovolumic hemodilution was not used in the perioperative period,only recycled autotransfusion technology was used.The differences of hemoglobin(HB),hematocrit(HCT),intraoperative bleeding volume,three-day flow volume,hospital stay,three-day allogeneic leukocyte removal,fresh frozen plasma transfusion volume,recessive blood loss volume,total amount and recovery rate of intraoperative self blood were compared and analyzed.Results: 1.The total amount and recovery rate of self blood in the experimental group were slightly higher than that in the control group,Although the difference was not statistically significant(all P > 0.05);2.The total amount of allogeneic red blood cell transfusion in the whole perioperative period,the amount of allogeneic red blood cell transfusion in the three days after operation and the rate of allogeneic red blood cell transfusion in the three days after operation in the experimental group were lower than that in the control group,the difference was statistically significant(all P < 0.05);3.The amount of recessive RBC loss in the experimental group was lower than that in the control group,The difference was statistically significant(P < 0.05);4.The length of stay in the experimental group was shorter than that in the control group,the difference was statistically significant(P < 0.05).Conclusion: 1.On the basis of intraoperative use of recycled autohemodilution,combined use of acute isovolumic hemodilution does not significantly reduce the blood transfusion volume and recovery rate of autohemodilution in patients undergoing hip joint revision;2.Intraoperative use of acute isovolumic hemodilution can maintain a higher level of platelets and plasma proteins after operation,thus significantly reducing the concealment within three days after hip joint revision 3.Acute normovolemic hemodilution significantly reduced the total transfusion volume and transfusion rate of allogeneic red blood cells in the experimental group during the whole perioperative period,among which the transfusion volume and transfusion rate of allogeneic red blood cells in the three days after operation were significantly reduced,thus significantly shortened the hospital stay of patients in the experimental group. |
