| Objective:Based on the study of CT imaging findings of melioidosis pneumonia cavity,this article explore the correlation between melioidosis pneumonia and mycobacterium tuberculosis,staphylococcus aureus,klebsiella pneumoniae pneumonia infection and cavitation caused by lung cancer,which aim to improve the ability of early diagnosis and treatment of melioidosis pneumonia,to avoid delay treatment of melioidosis pneumonia,and to reduce mortality in patients with melioidosis pneumonia.Methods:We collected 147 patients with pulmonary cavity diagnosed from The First Affiliated Hospital of Hainan Medical University and The Second Affiliated Hospital of Hainan Medical University.There were 48 cases of melioidosis pulmonary cavity,58 cases of pulmonary tuberculosis cavity,24 cases of lung cancer cavity 7 cases of staphylococcus aureus pneumoniae cavity and 10 cases of klebsiella pneumoniae pneumonia cavity.Retrospective analysis method was used to systematic analysis basic data of patients including sex,age,satellite lesions around the cavity,thickness of cavity wall,and cavity content including mural nodule,liquid flat inside,spiculati--on,halo sign,mediastinal or hilar lymph node enlargement,multiple air cystic caviys,whether there were air sacs in lung field and stalactite sign after oblique fissure of lung.Results:1.The male to female ratio of melioidosis pneumoniae cavity was 11,tubrculous pulmonary cavity was 4.8,lung cancer cavity was 3,staphylococcus aureus pneumoniae cavity was 6,klebsiella pneumoniae pneumonia cavity was 9.There was no statistical significance difference between the sexes of pneumoidosis pneumoniae cavity and other four diseases(P>0.05).There was a significant differenc in the age between melioidosis pneumoniae cavity and lung cancer cavity(P<0.05).There was a significant difference in the age between melioidosis pneumonitis cavity and klebsiella pneumoniae pneumonia cavity(P<0.05).2.There were significant differences in satellite lesions,multiple air cystic cavitys and halo signs between melioidosis pneumonia cavity signs and tuberculosis lung cavity signs(P <0.05).3.The cavity signs of melioidosis pneumonia and lung cancer were statistically significant in relation to mediastinal or hilar lymph node enlargement,mural nodule,spiculation around the cavity and multiple air cystic cavitys(P<0.05).4.There was significant difference between melioidosis pneumonitis cavity and staphylococcus aureus pneumoniae cavity in term of multiple air cystic cavitys(P< 0.05),and there was statistical significance in pneumtocele(P<0.05).5.There was significant difference between melioidosis pneumonia cavity and klebsiella pneumoniae pneumonia in multiple air cystic cavitys and halo sign(P <0.05),but statistical significance in stalactite(P < 0.05).6.The diagnostic sensitivity and specificity of melioidosis pneumonia in multiple air cystic cavitys are 87.5% and 94.9%,and the specificity of satellite lesions in the diagnosis of pulmonary tuberculosis is 79.3%.The specificity of mediastinal or hilar lymph node enlargement in the diagnosis of lung cancer is 78.3%,and the specificity of pneumtocele in the diagnosis of staphylococcus aureus pneumonia is 97.1%.The stalactite sign after oblique fissure of lung with a specificity of 100% for klebsiella pneumoniae pneumonia.Conclusions:In the comparison of CT cavity signs of melioidosis pneumonia with tuberculous pneumonia,lung cancer,staphylococcus aureus pneumonia and klebsiella pneumoniae pneumonia,it indicate that multiple air cystic cavitys were provided with diagnose melioidosis pneumonia specifically.CT cavity signs have great significance in the diagnosis and differential diagnosis of melioidos is pneumonia,tuberculous pneumonia,lung cancer,Staphylococcus aureus pneumonia and klebsiella pneumoniae pneumonia,it provides important basis for clinicians in the diagnosis and differential diagnosis of pulmonary diseases,which is worth popularizing and applying. |
