The Clinical Effect Of Jianpi Lidan Formula Combined With PTCD In The Treatment Of Malignant Obstructive Jaundice

Objective:to observe and analyze the clinical efficacy of Jianpi Lidan decoction combined with percutaneous transhepatic cholangial drainage(PTCD)in the treatment of malignant obstructive jaundice,and to explore the combination of traditional Chinese medicine and modern medicine for better efficacy.Methods:from October 2017 to September 2019,64 patients with malignant obstructive jaundice were randomly divided into Observation Group(PTCD+Jianpi Lidan decoction)and Control Group(PTCD),the clinical efficacy of Jianpi Lidan decoction combined with PTCD in the treatment of malignant obstructive jaundice was evaluated by observing the changes of liver function(AKP,ALT,AST,TBIL,DBIL),blood routine(GRA%,WBC),symptoms and signs of traditional Chinese Medicine(TCM)and systemic functional status.Results:①The recovery of Liver Function:The AKP,ALT,AST,TBIL and DBIL of the two groups recovered significantly after treatment,and there was no significant difference between the two groups at 3 days after treatment,the liver function(AKP,ALT,AST,TBIL,DBIL)were tested,p>0.05,there was no significant difference between the two groups at 7 days,p<0.05,there was a significant difference,the results showed that the recovery of liver function indexes in the Observation Group was superior to that in the control group,and the recovery of liver function indexes in the Observation Group was superior to that in the control group.②Blood routine(GRA%,WBC):patients in the two groups had inflammatory reaction after PTCD,7 days after the peak,through statistics found that patients in the observation group GRA%,WBC compared with the control group increased less and recovered more quickly,however,GRA%and WBC of the patients in both groups returned to the normal value after 14 days of treatment,suggesting that Jianpi Lidan decoction is effective in preventing and treating postoperative infection and fever.③Evaluation of the curative effect of Tcm Syndrome:through treatment,the scores of the two groups decreased,the total effective rate of the final observation group(PTCD+Jianpi Lidan Fang)was 90%,the total effective rate of the control group(PTCD)was 50%,there was a statistical difference,it is suggested that the improvement of TCM syndrome of the patients with malignant obstructive jaundice in the observation group is obviously better than that of the Control Group,and the curative effect of the patients in the observation group is obviously better than that of the control group in the improvement of abdominal distension,appetite and listlessness,it is suggested that Jianpi Lidan decoction is effective in relieving abdominal distension,improving appetite and enhancing physical strength.④Functional Status Evaluation:Through treatment,the KPS scores of the two groups were improved,the improvement rate of KPS scores of the observation group(PTCD+Jianpi lidan prescription)was 96.67%,and that of the Control Group(PTCD)was 62.50%,it indicates that Jianpi Lidan decoction has significant advantages in improving the systemic functional status of patients with malignant obstructive jaundice.Conclusion:In the treatment of malignant obstructive jaundice,PTCD combined with Jianpi Lidan decoction combined with traditional Chinese and Western medicine has better effect in reducing jaundice and promoting the recovery of liver function than PTCD alone,however,the former has obvious advantages in improving functional status,systemic symptoms,especially abdominal distension,loss of appetite,lassitude and weakness,as well as prevention and treatment of complications,it fully proves the auxiliary significance of Jianpi Lidan decoction in the treatment of malignant obstructive jaundice,and embodies the advantages of integrated traditional Chinese and Western medicine.
