Clinical Investigation Of Myasthenia Gravis Syndrome Type In Traditional Chinese Medicine And MGFA Type In Western Medicine

Objective:To provide theoretical support for the prevention and treatment of myasthenia gravis by investigating the clinical characteristics of myasthenia gravis patients and analyzing the clinical distribution of TCM syndrome type and Western medicine MGFA type.Methods:In this study,the clinical data of 101 myasthenia gravis patients(including general information,relevant medical history,physical and laboratory examination)were collected,and the traditional Chinese medicine syndrome type and Western medicine MGFA type were determined,and the statistical software spss22.0 was used for data statistical analysis.Results:(1)Among the 101 inpatients with myasthenia gravis,the most common age group was the elderly group(58 cases,57.43%),the rest were the middle-aged group(31 cases,30.69%),the youth group(7 cases,6.93%),and the children group(5 cases,4.95%).(2)Among 54 patients with myasthenia gravis.45(83.33%)were positive for acetylcholine receptor antibody,and the positive rate of acetylcholine receptor antibody in MG patients with thymoma was 100%.There was no significant difference in the distribution of TCM syndrome type and Western medicine MGFA type in patients with different acetylcholine receptor antibody(P>0.05).(3)Among 101 myasthenia gravis patients,28(27.72%)had thymoma and 73(72.28%)had no thymoma.(4)Among 101 cases of myasthenia gravis,the most common type of TCM syndrome is spleen and stomach deficiency(63 cases,62.4%),the rest are spleen and kidney deficiency(20 cases,19.8%),liver and kidney yin deficiency(13 cases,12.9%),atmospheric depression(5 cases,4.9%).(5)Among 101 patients with myasthenia gravis,the most common type of MGFA in western medicine was type Ⅱa(42 cases,41.59%),the others were type Ⅰ(29 cases,28.71%),type Ⅱ b(23 cases,22.77%),type Ⅳ b(2 cases,1.98%),type Ⅴ(2 cases,1.98%),type Ⅲa(1 case,0.99%),type Ⅲb(1 case,0.99%),type Ⅳb(1 case,0.99%).(6)Deficiency of spleen and stomach is the most common type of TCM in all age groups,including children group(80.0%,4/5),youth group(85.7%,6/7),middle-aged group(61.3%,19/3 1),old group(58.6%,34/58);deficiency of liver and kidney yin and deficiency of spleen and kidney are the most common in middle-aged and old group.There was no significant difference in the distribution of TCM syndromes and Western medicine MGFA in different age groups(P>0.05).(7)The distribution of MGFA in different genders was statistically significant(P<0.05).In female patients,MGFA was mainly classified as type Ⅱ a(44.9%,22/49),type Ⅱ b(30.6%,15/49),type Ⅰ(18.4%,9/49);in male patients,type Ⅰ(38.5%,20/52),type Ⅱ a(38.5%,20/52),type Ⅱ b(15.4%,8/52).However,there was no significant difference in the distribution of TCM syndrome types between different genders(P>0.05).(8)There were more thymomas in middle-aged and old patients(46.4%,13/28).There was no significant difference in the distribution of thymoma in different age groups(P>0.05).(9)The main TCM Syndromes of thymoma patients were spleen and stomach deficiency(53.6%,15/28),spleen and kidney deficiency(21.4%,6/28),liver and kidney yin deficiency(17.9%,5/28).There was no significant difference in the distribution of TCM syndromes and Western medicine MGFA in patients with different thymus conditions(P>0.05).(10)There was statistical significance in the distribution of Western medicine MGFA in different TCM syndromes(P<0.001).In patients with deficiency of spleen and stomach,MGFA in western medicine is mainly classified as type Ⅱ a(41.3%,26/63),type Ⅰ(30.2%,19/63);in patients with deficiency of liver and kidney yin,MGFA in western medicine is mainly classified as type Ⅱ a(53.8%,7/13),type Ⅰ(46.2%,6/13);in patients with deficiency of spleen and kidney,MGFA in western medicine is mainly classified as type Ⅱ a(45.0%,9/20),type Ⅱ b(30.0%,6/20);in patients with depression in the atmosphere Type V(40.0%,2/5)and type ⅣB(40.0%,2/5)were the main typesConclusion:(1)Among the 101 myasthenia gravis patients who participated in the survey,the elderly group was the most common,followed by the middle-aged group,the youth group and the youth group.(2)In this study,the positive rate of acetylcholine receptor antibody was 83.33%in myasthenia gravis patients,and 100%in MG patients with thymoma.(3)Type Ⅱ a,type I and type Ⅱ b are common types of MGFA in patients with myasthenia gravis.(4)The common western medicine MGFA classification of myasthenia gravis female patients is type Ⅱ a,type Ⅱ b;the common western medicine MGFA classification of male patients is type Ⅰ,type Ⅱa.(5)Myasthenia gravis with deficiency of spleen and stomach is the most common type of traditional Chinese medicine in all age groups.(6)There were more thymoma in the middle-aged and elderly patients,and the syndrome types of thymoma patients were mainly spleen and stomach deficiency,spleen and kidney deficiency,liver and kidney yin deficiency.(7)In patients with deficiency of spleen and stomach,MGFA in western medicine is mainly classified as type Ⅱ a and type Ⅰ;in patients with deficiency of liver and kidney yin,MGFA in western medicine is mainly classified as type Ⅱ a and type Ⅱ b;in patients with deficiency of spleen and kidney,MGFA in western medicine is mainly classified as type Ⅱ a and type Ⅱ b;in patients with depression of atmosphere,MGFA in western medicine is mainly classified as type Ⅴand type Ⅳb.
