| BackgroundThyroid nodules are common clinical thyroid diseases.Thyroid cancer has become one of the most common malignant endocrine tumors.With the improvement of people’s living standards,health awareness is becoming stronger and stronger.The incidence rate is increasing year by year.It has become an important problem that puzzles the health of our residents.In recent years,contrast-enhanced ultrasound has been used in the differential diagnosis of benign and malignant thyroid nodules.It can dynamically show the microcirculation and blood perfusion state of thyroid nodules and surrounding tissues,and show the micro blood flow distribution and difference between the lesions and surrounding normal or abnormal thyroid tissues One way to differentiate benign from malignant thyroid nodules.At present,there is no relevant research on the differential diagnosis of benign and malignant by observing the characteristic variables of contrast-enhanced ultrasound.ObjectiveTo explore the value of characteristic variables of contrast-enhanced ultrasound in the differential diagnosis of benign and malignant thyroid nodules.MethodsThe clinical data of 341 thyroid nodules in 312 patients who underwent contrast-enhanced thyroid ultrasound examination in our hospital from December 2016 to December 2017 were retrospectively analyzed.According to the pathological results,they were divided into benign group(105 nodules)and malignant group(236 nodules).The general information and contrast-enhanced ultrasound characteristic variables of patients were compared,and the logistic model was established with different contrast-enhanced ultrasound characteristic variables The regression model was compared and the receiver operating characteristic curve(ROC)was drawn to evaluate the effectiveness of the logistic regression model with contrast-enhanced ultrasound in the differential diagnosis of benign and malignant thyroid.ResultsThere was no significant difference in the general data of benign and malignant nodules(P > 0.05);there were significant differences in perfusion defect,uniformity,annular enhancement and peak intensity between benign group and malignant group(P ≤0.05),while there was no significant difference in other characteristic variables(P ≥ 0.05)Logistic regression analysis was used to analyze the characteristic variables of contrast-enhanced ultrasound.Perfusion defect,homogeneity and peak intensity were the influencing factors for the diagnosis of benign and malignant thyroid nodules.Based on the logistic regression model,logit(P)=-4.763 + 2.181 × uniformity + 2.747 ×perfusion defect + 4.158 × peak intensity was established.The likelihood ratio test was χ2 = 106.437,P ≤ 0.001。The ROC curve drawn by this regression model showed that the area under the curve was 0.962±0.28,P≤0.001,and 95 % confidence interval was(0.942,0.982),and the sensitivity,specificity and accuracy rate of differential diagnosis of benign and malignant thyroid nodules were 86.87 %,87.62 % and 87.10 %.ConclusionsPerfusion defect,inhomogeneous enhancement and low contrast enhancement were statistically significant in the differential diagnosis of benign and malignant thyroid nodules.The logistic regression model based on the three contrast-enhanced ultrasound signs had good efficacy in the differential diagnosis of benign and malignant thyroid nodules. |
