PurposeGynecologic diseases,including uterine fibroids,cervical cancer and ovarian cancer,can be treated by laparoscopy these days.Gynecologic Oncology Department of our hospital mainly treats various gynecological tumors.At present,the rate of endoscopic surgery accounts for more than eighty percent of the total operative volume of the Department.However,the incidence of postoperative nausea and vomiting is significantly higher than that of other surgical patients due to various reasons.Postoperative nausea and vomiting not only cause obvious discomfort in the patient and may lead to complications such as aspiration pneumonia resulting in poor quality of early postoperative recovery,but also aggravate the patient’s dissatisfaction with the treatment and the increase in the cost of treatment.This study focuses on the therapeutic effect of transcutaneous acupoint electrical stimulation on postoperative nausea and vomiting at different time points,so as to further observe the improvement of the quality of early postoperative recovery.Method156 cases of gynecologic oncology department undergoing elective laparoscopic surgery.The random number method was used to divide to the A,B and C 3 groups with 52 cases in each group.Two patients were excluded from group B and C,the final number of experiments was 152.The three groups of anesthesia methods were the same.The group A received electrical stimulation at both sides of the Neiguan and Zusanli points for 30 minutes before operation anesthesia.Group B was treated with electrical stimulation at both sides of the Neiguan and Zusanli points for 30 minutes after operation anesthesia.In group C,only electrodes were placed on both sides of the Neiguan and Zusanli points,and they were not connected to the therapeutic apparatus as control group.Observation record:1.The incidence of nausea and vomiting after operation in each time period(0-2H,2-6H,6-12H,12-24H),During the same period,repeated nausea and vomiting occurred only record once.Record the use of analgesics,contain the type and dosage of drugs.2.Record the VAS score for rest and cough state in each period(12H,24H,36H,48H after operation).Points 0 is pain free,10 is intolerable pain,and the following 4 points are effective for pain relief.3.After 48 hours,the patients and their families were carefully inquired and the QoR-40 scale was filled to observe the early recovery quality of the patients.ResultCompared with group C,patients in group A and B had better physical comfort and emotional state 48 hours after surgery(P<0.05),and had a higher total score for QoR-40,but there was also no statistical difference between the Group A and Group B.The incidence of postoperative nausea and vomiting in Group A and Group B was significantly lower than that in Group C respectively(p<0.05).And there was also no statistical difference between the Group A and Group B.Compared with Group C,the VAS scores for Rest and Cough state of Group B and Group C were significantly decreased after operation(p<0.05),and there was also no statistical difference between the Group A and Group B.ConclusionPercutaneous acupoint electrical stimulation has higher quality of early recovery,lower incidence of nausea and vomiting,and less postoperative pain than the patients without this method.However,there was no significant difference in the improvement of the quality of early postoperative recovery between preoperative and postoperative point stimulation.
