Objective:Uric acid in the human body comes from dietary intake and nucleotide catabolism,and is mainly excreted by uric acid transporters in the kidney and intestine.The increased production and/or reduced excretion of uric acid will lead to the increase of serum uric acid,leading to the occurrence of hyperuricemia(HUA),and further affect the pathological changes or functional changes of related organs.The occurrence of hyperuricemia is also influenced by genetic factors,of which the ABCG2 Q141 K mutation is one of the most frequently studied loci.This study will analyze the relationship between the increase of serum uric acid content and the indexes of liver and kidney function and serum lipid,and preliminarily explore the association between HUA and ABCG2 Q141 K mutation,so as to provide certain laboratory basis for the study of the etiology and pathogenesis of HUA.Method:706 inpatients in the Third People’s Hospital of Hubei Province from March to June 2019 were selected as the research subjects.Fasting venous blood was collected in the morning,serum uric acid levels,biochemical indexes of liver function(alanine aminotransferase(ALT),aspartate aminotransferase(AST),total protein(TP),albumin(ALB),total bilirubin(TBIL)and direct bilirubin(DBIL)),biochemical indexes of renal function(blood urea nitrogen(BUN)and serum creatinine(Scr))and serum lipid indexes(serum triacylglycerol(TG),serum total cholesterol(TC),low-density lipoprotein cholesterol(LDLC)and high-density lipoprotein cholesterol(HDLC))was detected in all specimens according to routine laboratory procedures.Genomic DNA was extracted from the whole blood of EDTA-K2 anticoagulant,and the ABCG2 gene fragment was amplified by polymerase chain reaction(PCR).The nucleotide sequence information of this segment was detected by sequencing technology,and the mutation of ABCG2 Q141 K in peripheral blood genomic DNA of HUA patients was analyzed.According to the diagnostic criteria of HUA,the subjects were divided into HUA group and normal serum uric acid group(NUA).The differences of liver function parameters,renal function parameters and lipid parameters between HUA group and NUA group were compared,and the correlations between serum uric acid and liver function indexes,renal function indexes and lipid indexes were detected.Analyze the influence of changes in serum uric acid content on liver and kidney function,and the influence of changes in lipid content on HUA.Results:1.The parameters ALT,AST,TBIL and DBIL of HUA group were significantly higher than those of NUA group,while the parameters ALB were opposite(P<0.05).Except for TP,serum uric acid was positively correlated with ALT,AST,TBIL and DBIL,and negatively correlated with ALB(P<0.05).In subjects of the same sex,with the increase of serum uric acid content,the parameters ALT,AST,TBIL and DBIL gradually increased,while the ALB parameters were opposite(P<0.05).2.The parameters of Scr in HUA group of different genders were significantly higher than those in NUA group,but the estimated glomerular filtration rate(e GFR)was opposite(P<0.05).EGFR was negatively correlated with serum uric acid(P<0.05),increased serum uric acid content was a risk factor for chronic kidney disease(CKD),and every 1μmol/L increase in serum uric acid content was associated with a1.015 and 1.020 fold increase in the incidence of CKD in male and female,respectively(P<0.05).3.The parameters TG and CHOL in male HUA group were significantly higher than those in NUA group,while the parameters of HDLC were opposite(P<0.05).The parameters TG,CHOL and LDLC in female HUA group were significantly higher than those in NUA group(P<0.05).In male,serum uric acid was positively correlated with TG and CHOL,but negatively correlated with HDLC(P<0.05).In female,serum uric acid was positively correlated with TG,CHOL and LDLC(P<0.05).Hypertriglyceridemia,hypercholesterolemia and Low high-density lipoprotein cholesterolemia are risk factors for male HUA,while hypertriglyceridemia and hypercholesterolemia are risk factors for female HUA(P<0.05).4.ABCG2 Q141 K mutation occurred in the peripheral blood genomic DNA of 38.9% HUA patients,and there were different ABCG2 Q141 K genotypes.Conclusion: The increase of serum uric acid content caused changes in clinical liver function index data(ALT,AST,TBIL,DBIL and ALB)and renal function index data(e GFR and Scr).Controlling serum uric acid level in the normal reference range may slow down the development of liver and renal function injury.The change of lipid content is closely related to the occurrence of HUA,and there is a gender difference.Hypertriglyceridemia,hypercholesterolemia and Low high-density lipoprotein cholesterolemia are risk factors for male HUA,while hypertriglyceridemia and hypercholesterolemia are risk factors for female HUA.ABCG2 Q141 K mutation was found in whole blood genomic DNA of HUA patients,but further study was needed with expanded sample size.
