The World Health Organization(WHO)describes telemedicine as "a practice of healthcare services used in interactive audiovisual devices and data communications,including providing healthcare services,diagnostics,education and transmission of healthcare data." With the explosive growth of Internet information and the outbreak of the epidemic,telemedicine has been rapidly developed.The new medical model not only enriches the way of seeing a doctor for patients,but also provides great convenience for patients.However,most of the telemedicine services are based on commercial platforms,and most of the public hospitals that cooperated with are third-class hospitals,which cannot fundamentally relieve the situation of medical resource shortage in China.At present,the development of the Internet presents the overflow of medical information on the Internet,and its induced effect enables patients to choose the way of medical treatment by themselves through the Internet.Therefore,the development of telemedicine provides new ideas for the realization of hierarchical diagnosis and treatment to a large extent.Hierarchy diagnosis and treatment is an important system for the reform of the medical and health system in China’s 14th Five-Year Plan,which mainly includes"minor illnesses go to the grass-roots level,and major illnesses go to the hospital".The development of the Internet can induce patients to solve problems through the Internet,so telemedicine will promote the development of hierarchical diagnosis and treatment.However,many online health platforms fail to include grass-roots medical and health institutions in the scope of cooperation.Based on this,this study believes that in order to promote hierarchical diagnosis and treatment,primary medical and health institutions should also be included in telemedicine services.Most of the previous studies focus on the telemedicine services of third-party commercial platforms,while there are not many researches on the telemedicine services of public hospitals.Based on this,this study takes the public hospital system as the research object to study the telemedicine service,hoping to explore the supporting role of telemedicine service to the hierarchical diagnosis and treatment and then effectively help patients form the awareness of "go to the grassroots for minor illnesses,go to the hospital for serious illnesses".Among the patients,young people accept new things quickly and are good at using the Internet to solve problems.In order to vigorously promote hierarchical diagnosis and treatment,we can combine telemedicine to achieve diversion and achieve the goal of hierarchical diagnosis and treatment by cultivating medical habits.Based on this,the main research question of this paper is:the influence mechanism of young people’s willingness to choose telemedicine services.Based on this research problem,the following two steps are mainly carried out:One is to explore the key mechanisms influencing young people’s adoption of telemedicine services based on an experimental approach.Our research is firstly carried out by interviewing different young people to understand the influencing factors of their current choice of telemedicine services and the channels through which they choose telemedicine services.Through the method of interviews and literature,the mechanism of influencing young people’s choice of telemedicine services was determined.Experimental variables were extracted to carry out experimental research,and the interactive influence of the experimental variables of hospital grade+disease type,hospital grade+privacy protection,and hospital grade+diagnosis and treatment synergy was explored.The second is to establish the influencing factor model of young people’s willingness to choose telemedicine services and verify it by questionnaire survey.By combining experiment variables with the existing literature to determine the influence factors of young people choosing remote medical service,we identified with technology acceptance and use model(UTAUT)as the theoretical basis,and through the way of questionnaire survey,using SPSS and Smart PLS to build the structural equation model to explore the young people use of remote medical service six main factors:how utilitarian performance expectancy,efficiency expectancy,perceived privacy risk,perceived care risk,IT self-efficacy,and diagnosis and medical synergy affect youth’s evaluation of expected access to care,perceived trust,and self-health management of telemedicine services,and ultimately determine intention to use.Through the study of the factors influencing young people’s willingness to choose telemedicine services,it will also provide guidance and suggestions for the implementation of hierarchical diagnosis and treatment in the future.Through experimental research,we draw the following conclusions:1)In the face of low-risk diseases,there was no significant difference in patients’choice of telemedicine services provided by third-class hospitals and community health service centers;For high-risk diseases,patients tend to choose telemedicine services provided by hospitals with higher grades.2)When the level of privacy protection was high,there was no significant difference between the choice of telemedicine services provided by third-class hospitals and community health service centers;However,when the degree of privacy protection was low,there was no significant difference in patients’ choice of telemedicine services provided by third-class hospitals and community health service centers.3)There was no significant difference between the services with high and low diagnosis and treatment synergies when the third class A hospitals provided telemedicine services;However,when the community health service center provided telemedicine services,patients tended to choose the services with high diagnosis and treatment synergy.Through the questionnaire survey,we have drawn the following conclusions:Utilitarian performance expectancy has a significant positive effect on expected access to care,perceived trust and self-health management.Efficiency expectancy had a significant positive effect on expected access to care and perceived trust,but had no significant effect on self-health management.Perceived privacy risk only had a significant negative correlation with perceived trust,but had no effect on expected access to care and self-health management.Perceived medical risk had no significant effect on the three dependent variables.IT self-efficacy can only be expected to be positively correlated with expected access to care,while the other two are not significant.Medical synergy had no significant effect on perceived trust,while the other two were significant.Expected access to care,perceived trust and self-health management were all positively correlated with intention to use.The innovation in this study is the investigation of the role of telemedicine services in hierarchical diagnostics and treatment in the context of "Internet+healthcare" from a hierarchical diagnostics and therapeutic perspective.At the same time,we also conducted empirical exploration through the combination of laboratory experiment and questionnaire survey,which opened up a new direction for the study of telemedicine services and hierarchical diagnosis and treatment.There are some limitations in this study.On the one hand,the selection range of experimental subjects is small,and a larger range of subjects can be selected in the future to understand the evaluation of different groups on telemedicine design factors.On the other hand,the degree of privacy protection as an experimental variable did not meet our expectations in our study,and the privacy protection policy may not be a good representative of the degree of privacy protection.In the future,we need to explore the variables that can represent the degree of privacy protection to improve the relevant studies.
