| Objective: To explore the related risk factors and prognosis analysis of traumatic coagulopathy in patients with severe multiple injuries.Methods:From 2014 to 2020,the First Affiliated Hospital of Xinjiang Medical University collected patients with multiple trauma and ISS score ≥16.According to INR ≥1.2,131 cases were divided into TIC group and non-TIC group.Comparing the gender,age,traumatic time,diastolic and systolic blood pressure,body temperature,red blood cell specific volume,blood p H,ISS score,APACHEII score,The clinical data of the two groups of patients,such as gender,age,hospital admission time,blood pressure,body temperature,hematocrit,PH value,ISS score,APACHEII score,and other clinical data were analyzed by univariate analysis,and then the independent risk factors affecting the occurrence of TIC were analyzed by binary logistic analysis.Analyze the influencing factors of the outcome of TIC patients.Results: Among the 131 patients with serve multiple trauma,46 were in the TIC group and 85 were in the non-TIC group.ISS score,APACHEII score,red blood cell,Hb,hematocrit,APTT,PT,fibrinogen,platelet,PH,BE,LAC,hypotension,hypothermia,the difference was statistically significant(P<0.05),ISS score,APACHEII score,PT,BE can be used as independent risk factors for invasive coagulopathy.The independent risk factors for death in the TIC group were MODS and DIC.Conclusion: The severity of injury,APACHEII score,PT and BE are independent risk factors for traumatic coagulopathy in patients with severe multiple injuries.In the TIC group,MODS and DIC are independent risk factors for death.Therefore,careful evaluation of ISS score,APACHEII score,PT and BE after admission to patients with multiple injuries has clinical significance in preventing patients from developing TIC;for TIC patients,the presence of MODS or DIC indicates a higher risk of death. |
