Comparative Study Of Complete Thoracoscopy And Median Thoracotomy In The Treatment Of Atrial Septal Defect

Objective : By comparing the clinical data of complete thoracoscopy with median thoracotomy,To evaluate the clinical efficacy of total thoracoscopic surgery for atrial septal defects(ASD).Methods:To collect and the clinical data of 134 patients with atrial septal defect treated in the department of cardiac surgery of the first affiliated Hospital of Xinjiang Medical University from October 2018 to October 2020,The patients were divided into two groups according to different operation methods :50 cases in complete thoracoscopic group and 84 cases in median thoracotomy group.For the two groups of patients,the operation time,cardiopulmonary bypass time,ventilator auxiliary time,ICU residence time,bleeding volume,use of blood products,postoperative hospitalization days,complications,total hospitalization costs and other clinical data were statistically analyzed.Results:All patients underwent surgery successfully,and no serious complications occurred during perioperative period.There was no significant difference between the two groups in general condition,ICU residence time,ventilator assisted time,postoperative complications and total hospitalization cost(P>0.05).Complete thoracoscopic group in cardiopulmonary bypass time longer than the median thoracotomy group,P<0.05,however,the amount of bleeding,the number of cases of blood products and postoperative hospitalization days were shorter than the median thoracotomy group,P<0.05,the difference was statistically significant.Conclusion:Compared with median thoracotomy,complete thoracoscopic atrial septal defect repair increased intraoperative cardiopulmonary bypass time,but did not increase surgical complications.Complete thoracoscopic atrial septal defect repair is safe and reliable.Less trauma,beautiful incision,less bleeding,saving blood,short postoperative hospitalization days,can be used as a priority.
