Objective: The presence of pathogenic bacteria in dental plaque can cause inflammatory,destructive disease in the supporting tissue of the teeth,which eventually develops into periodontitis.As a common endocrine metabolic disease,type 2 diabetes mellitus(T2DM)is characterized by insulin resistance and decreased insulin sensitivity.The two diseases share some similar pathogenetic characteristics,and both exhibit enhanced immune inflammatory responses in the presence of similar biological inflammatory mediators.In this paper,the expression of Chemerin(CHEM)in gingival crevicular fluid of patients with type 2 diabetes mellitus(T2DM)complicated with periodontitis and its correlation with the disease were detected.At the same time,we analyzed the correlation between the expression level of CHEM in gingival crevicular fluid and periodontal index,blood glucose related laboratory indexes,and further understood the correlation between the two diseases from the level of molecular biology,so as to provide new ideas for the prevention and treatment of T2 DM patients with periodontitis in the future.Methods: From November 2019 to October 2020,a total of 79 patients were selected and divided into four groups.They were from the Department of Endocrinology and physical examination center of Liaoning Provincial People’s Hospital.While fully respecting the wishes of patients,the inclusion criteria and exclusion criteria were strictly followed.19 patients with type 2 diabetes were selected as type 2 diabetes group(T2DM group),52.95±10.16,12 males and 7females,20 patients with type 2 diabetes complicated with periodontitis group(P+T2DM group),55.50±8.42,9 males and 11 females,18 patients with periodontitis were selected as the periodontitis group(P group),53.61±11.63,8males and 10 females,There were 22 healthy controls group(H group,49.40±10.41),14 males and 8 females.The general condition(Age,gender,smoking,periodontitis stage,body mass index(BMI),course of T2DM),Probing depth(PD),clinical attachment loss(CAL),bleed-ing index(BI),simplified calculus index(CI-S),simplified soft scale index(DI-S),fasting blood glucose(FPG),glycosylated hemoglobin(Hb A1c)were recorded from each group.Gingival crevicular fluid samples of 4 groups were collected by moisture absorption paper tip weighing method.and the expression of CHEM in gingival crevicular fluid was detected by enzyme-linked immunosorbent assay(ELISA).the correlation between CHEM expression and periodontal index,FPG,Hb A1 c was analyzed by SPSS 23.0.Results:1.The expression level of CHEM in gingival crevicular fluid of P+T2DM,T2 DM and P groups was significantly higher than that in H group,and the difference was statistically significant;The expression level of CHEM in gingival crevicular fluid in P+T2DM group was significantly higher than that in P group,and the difference was statistically significant;The expression level of CHEM in gingival crevicular fluid in P+T2DM group was significantly higher than that in T2 DM group,and the difference was statistically significant.2.The expression level of CHEM in gingival crevicular fluid of P group was positively correlated with PD,CAL,BI,CI-S and DI-S(P<0.05).The expression of CHEM in gingival crevicular fluid in T2 DM group was positively correlated with FPG and Hb A1 c.The expression of CHEM in gingival crevicular fluid of P+T2DM group was significantly positively correlated with Hb A1c(P<0.01),and positively correlated with PD,CAL,BI,CI-S,DI-S,FPG(P<0.05).3.Multiple linear stepwise regression analysis was conducted with Chem as the dependent variable,and periodontal indexes,FPG and Hb A1 c as independent variables were included in the regression analysis.The results showed that in group P,CAL(P < 0.05,β = 0.51)and CI-S(P < 0.05,β = 0.45)were the important factors influencing the change of CHEM level;In group T2 DM,FPG(P < 0.05,β = 0.57)were the important factors influencing the change of CHEM level;In group T2 DM +P,Hb A1c(P < 0.05,β = 0.52)and AL(P < 0.05,β = 0.46)were the important factors influencing the change of CHEM level.4.The analysis of periodontal index showed that PD,CAL,BI,CI-S,DI-S and MT in T2 DM + P group and P group were higher than those in H group,and the difference was statistically significant.PD,CAL and CI-S,in P+T2DM group were higher than those in P group,and the difference was statistically significant.The analysis of blood glucose related laboratory indexes showed that FPG and Hb A1 c in P+T2DM group were significantly higher than those in T2 DM group,the difference was statistically significant.Conclusion:1.The expression level of chemerin in gingival crevicular fluid of periodontitis group was significantly higher than that in healthy control group,these results suggest that chemerin may be involved in the occurrence and development of periodontitis.2.The expression level of chemerin in gingival crevicular fluid of periodontitis with type 2 diabetes mellitus group was significantly higher than that of the other three groups.These results suggest that chemerin are related to the development of periodontitis and type 2 diabetes.
