| Objective: To study the correlation between serum trace elements levels and diabetes II and its chronic complications in the elderly in Northeast China.By detecting the content of serum trace elements and measuring the oxidative and antioxidant indexes of the body, the levels of serum trace elements, oxidation and antioxidant of elderly diabetes patients with different degrees of complications were analyzed, the correlation between trace elements and oxidative stress indexes was discussed, and the risk factors for the severity of diabetes disease were determined. It provides theoretical basis and clinical application basis for prevention of senile diabetes and its complications, and supplementary treatment of trace elements in senile diabetes.Method: Collect the inpatients, who were definitely diagnosed as diabetes by Department of Geriatrics, the first hospital of Jilin University from January 2019 to September2020,as the experimental group,at the same time collect non diabetic patients in the same period as the control group. With the self-made questionnaire to investigate the basic information, state of an illness, anamnesis, recent medication & health products of the research objects, the levels of trace elements iron(Fe),magnesium(Mg),zinc(Zn),fasting blood glucose(FBG),glycosylated hemoglobin(HbAlc),cholesterol fat(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL),high density lipoprotein cholesterol(HDL)and other related biochemical indexes were measured.Four oxidative stress indexes such as the activities of nitric oxide(NO),malondialdehyde(MDA),superoxide dismutase(SOD)and total antioxidant capacity(T-AOC)were determined by the kit.Analyze the differences of serum trace elements, blood glucose, blood lipid and antioxidant index levels;analyze the correlation between trace elements and oxidative stress index,as well as in the case of combination of different complications, the situation of serum trace elements, blood glucose and antioxidant indexes in elderly patients with diabetes mellitus;analyze the risk factors of disease severity in elderly patients with diabetes.Results:(1)Compared with the control group, the levels of fasting blood glucose,TG,LDL,HbAlc,Mg,NO,SOD,T-AOC and Zn,MDA,in the experimental group were different, which has the statistically significance(P < 0.05).(2)In the experimental group, there were significant differences in fasting blood glucose,HbAlc,MDA,NO,SOD,T-AOC and Mg,Zn,contents among the elderly patients with different degrees of complications(P < 0.05).(3)The results of correlation analysis between serum trace elements and oxidative stress in elderly diabetic patients in the experimental group showed that there were significant differences in the content of Fe,SOD,T-AOC,MDA and Zn,in the experimental group(P <0.05),the correlation coefficients were 0.180,0.193,-0.146,0.147respectively;the content of Mg was significantly different from that of SOD,and it has the statistically significance(P <0.05),and the correlation coefficient was 0.147.The content of Zn was significantly different from that of SOD,T-AOC,MDA and NO,and it also has the statistically significance(P <0.05),and the correlation coefficients were 0.616,0.575,-0.636 and-0.616,respectively.(4)Univariate logistic analysis was used to analyze the risk factors of disease severity in elderly patients with diabetes. The results showed that age, course of disease, fasting blood glucose,LDL,HbAlc,MDA,NO,SOD,T-AOC and Zn were significantly different from the severity of diabetes(P < 0.05).(5)Multivariate logistic regression was used to analyze the influencing factors of severity of diabetes in the elderly. The results showed that the course of disease, fasting blood glucose,HbAlc,MDA,SOD and Zn were the influencing factors of the severity of diabetes in the elderly.Conclusion:(1)The levels of MDA and NO in the elderly diabetic patients were higher than those in the normal control group, while the levels of Mg,Zn,SOD and total antioxidant capacity in the elderly diabetic patients were lower than those in the control group.(2)The levels of serum trace elements and oxidative stress in elderly patients were different among the elderly patients with diabetes mellitus complicated with different degrees of complications.(3)There is a correlation between serum trace elements and oxidative stress in elderly diabetic patients. The content of Fe was positively correlated with Zn,SOD and T-AOC,while the content of Fe was negatively correlated with Cu and MDA;the content of Mg was positively correlated with SOD;the content of Zn was positively correlated with SOD and T-AOC,and the content of Zn was negatively correlated with MDA and NO.(4)In multivariate logistic analysis, the course of disease, fasting blood glucose,HbAlc and MDA were risk factors for the severity of diabetes in the elderly;Zn and SOD were protective factors for the severity of senile diabetes.(5)Maintaining the level of trace elements in elderly patients with diabetes can resist the oxidative stress reaction of the body,so as to achieve the purpose of adjuvant treatment of elderly diabetes and its chronic complications. |
