| Objective:The development of elastic intramedullary nail technique and fracture fixation concept provide an option for the treatment of forearm shaft fractures in adolescents.This study retrospectively analyzed the clinical efficacy of closed reduction and elastic intramedullary nail(TEN)fixation and open reduction and plate fixation in the treatment of forearm shaft fractures in adolescents,and compared the clinical efficacy of the two surgical methods in the treatment of forearm shaft fractures in adolescents.Materials and Methods:All cases of adolescent forearm shaft fractures treated with elastic intramedullary nail fixation and plate internal fixation from May 2018 to May 2020 in our hospital were included in this study(a total of 36 cases).They were divided into two groups according to the treatment methods: 18 patients in the elastic intramedullary nail group(TEN),11 males and 7 females,10–16 years,with an average age of(12 ± 1.53)years.There were 18 patients in plate group,among whom there were 10 males and 8 females,10-16 years,with mean one of(13 ± 1.38)years..In the elastic intramedullary needle group,13 cases were type A and 5 cases were type B.There were 11 cases of type A and 7 cases of type B in steel plate group.After the preoperative examination was completed,the forearm fractures were treated by the two surgical methods respectively.The clinical data of the two groups,such as the intraoperative time,intraoperative blood loss,postoperative fracture healing time,incidence of complications and forearm rotation function,were recorded in detail.Graceand Eversman scoring criteria were used to score the function of all patients after surgery,and all patient data were processed and analyzed using SPSS 23.0 statistical software.Result:All patients were followed up for an average of 17 months.The follow-up results showed that in terms of complications,2 patients had tail irritation in the elastic intramedullary nail group,1 patient had delayed fracture healing in the plate group,and the other patients had no complications.The intraoperative time,intraoperative blood loss,fracture healing time and incision length in elastic intramedullary nail group were(48 ± 11.94)minutes,(41 ± 8.09)ml,(32 ± 6.42)weeks and(4 ± 1.87)cm;the intraoperative time,intraoperative blood loss,fracture healing time and incision length in plate group were(61 ± 10.56)minutes,(90 ±21.32)ml,(36 ± 8.31)weeks and(9 ± 2.09)cm.Elastic intramedullary nail group was superior to plate group in the above aspects.And the difference between the two groups was statistically significant(P< 0.05).There was no significant difference between the elastic intramedullary nail group and the plate group in terms of postoperative forearm rotation function recovery.(P > 0.05).Conclusion:Both elastic intramedullary needle and steel plate are reliable,safe and effective methods for the treatment of adolescent forearm shaft fracture.Compared with plate internal fixation,elastic intramedullary nugget has the advantages of smaller incision,less bleeding and shorter fracture healing time,which can improve the clinical effect. |
