| BackgroundAfter decades of development, routine examinations of serum uric acid(SUA)and serum creatinine(SCr)was conducted in health checkup. The effect of health management intervention on on some knowledge and behaviors regarding hyperuricemia in physical examinees is poor due to their nagative attitudes as well as no physical symptoms.Secondly, the general health examination report does not show the estimated glomerular filtration rate(eGFR).Therefore, people with chronic kidney disease(CKD)can not be found in time.Patients with CKD are often found when they have other serious diseases and are hospitalized for examination. This study uses the large sample of physical examination data covering 21 prefecture-level cities from a hospital in guangzhou,Guangdong Province.Aims to analyze the HUA detection status and the correlation between HUA,lifestyle and CKD of the physical examination population.Provide health education and education basis for effective HUA prevention and treatment for physical examination population.Arouse the attention of health management agencies to CKD screening. And provide reference for the design of health management plan.ObjectiveThe purpose is to analyze the detection of HUA and its correlation with CKD in the physical examination population.Provide reference for HUA health education and early prevention of CKD for physical examination population..MethodThe physical examination data and lifestyle data of the health examinations were extracted, and statistical analysis was performed using Mann-Whitney Utest, two independent sample t-test,ChiSquare Test, binary logistic regression, multiple correspondence analysis, and 1:1 matching case-control study which Match according to the same gender,age±2years,and doing health checkup in the same year.To analyze the correlation between HUA with common chronic kidney diseases in the physical examination populations, and explore the influence of the lifestyle habits of the physical examination populations on the incidence of CKD.Results1.Tthe total detection rate of Hua was 35.8%(95%CI:35.5%-36.1%)in 2017.The physical examination populations with HUA has a higher detection rate of fatty liver(FL),and dy slipidemia. In the binary logistic regression model,HUA is related to gender, age,BMI,fatty liver, and dyslipidemia. In the middle-aged and elderly physical examination population,HUA is associated with obesity,FL,dyslipidemia, hypertension, and hyperglycemia. The ORs and 95%CI of female examinees were 2.29(95%CI:1.96-2.67),2.27(95%CI:2.04-2.53),1.29(95%CI:1.15-1.43),1.29(95%CI:1.15-1.43),0.78(95%CI:0.64-0.95),respectively.While in male examinees, the ORs and 95%CIwere 1.76(95%CI:1.59-1.94),1.68(95%CI:1.58-1.80),1.45(95%CI:1.36-1.54),1.25(95%CI:1.16-1.34),0.49(95%CI:0.44-0.54),respectively.All P<0.05.2.People with CKD is more likely to have metabolic syndrome,HUA,obesity and dyslipidemia. After adjusting for confounding factors,HUA is independent risk factors for CKD(OR=1.29).While those who skipped breakfast≥3 times a week, sleep efficiency ≤75%,and regularly consumed baked goods were at higher risk of CKD;those who had adequate intake of oil, aquatic products, soybeans and nuts were at lower risk of CKD.Conclusions1.The key population for prevention and treatment of HU A is men with dyslipidemia, fatty liver,overweight/obesity,and hypertention..When carrying out health education for medical examiners, several cardiovascular disease factors often associated with HUA patients can be used as a starting point, emphasizing the importance of controlling HUA for disease prevention.2.Compared with non-HUA group,HUA people are more likely to merge CKD.As for lifestyle behaviors, consuming edible oil, aquatic products, soybeans and nuts properly are also associated with the lower risk of CKD.Further,it is better to take breakfast at least 4 times a week, eat less baked foods and improve sleep efficiency. The general examination result of the health examination provides the eGFR calculation result, which may help the examinee realize the risk of CKD.For health management strategies,it is recommended to provide advice for CKD patients in terms of lifestyle, such as diet and sleep. |
