Objective:To observe the clinical efficacy of hemorrhoids anti-inflammatory ointment gauze after internal sphincter release in anal fissures,and to explore its mechanism of treating postoperative complications and promoting wound healing,so as to verify the clinical application value of hemorrhoids anti-inflammatory ointment.Methods:A total of 80 inpatients who met the inclusion criteria in the Anorectal Department of the First Affiliated Hospital of Guangxi University of Traditional Chinese Medicine from March 2020 to December 2020 were selected.After admission,the operations were treated with anal fissure internal sphincter lysis.According to the principle of allocation,80 patients were divided into treatment and control groups,each with 40 cases.The treatment group started to use hemorrhoids anti-inflammatory ointment gauze after the bath disinfection on the 2nd postoperative day,and the control group started to use Kangtai ointment gauze after the bath disinfection on the 2nd postoperative day,once a day.In the form of quantitative scores,the two groups of patients undergoing anal fissure surgery were recorded before dressing change,and on the 3rd,7th,and 14th day after surgery,anal pain,anal edema,wound hemorrhage,exudative complications,and granulation tissue growth;The wound size of the two groups changed at the above time points;after discharge,the patients were instructed to return to the hospital for dressing change every day,to evaluate the total effective rate of the two groups on the 14th day after the operation,and to follow-up to record the average wound healing time.The above scoring data was statistically analyzed using SPSS22.0 statistical software.Results:A total of 77 patients in this clinical study were finally included in the observation,including 38 cases in the treatment group and 39 cases in the control group.There was no significant difference between the two groups of patients in general conditions such as gender,age,disease course,anal fissure staging,and anal inspection after statistical analysis(P>0.05).There was no significant difference in the observation indexes and wound size between the two groups before the first dressing change after operation(P>0.05).The treatment results of hemorrhoids anti-inflammatory ointment gauze strip dressing compared with Kangtai ointment gauze strip dressing change are as follows:1.Comparison of anal pain:There is a statistically significant difference in anal pain scores between the two groups(F=10.983,P=0.001),and on the 3rd,7th,and 14th day after surgery,the scores of anal pain in the treatment group are all low In the control group during the same period(P3,7,14=0.000);comparison within the group,the anal pain scores of the two groups decreased with the passage of treatment time,and the difference was statistically significant(F=514.400,P=0.000).2.Comparison of anal marginal edema:comparison of scores of marginal edema between the two groups was statistically significant(F=5.800,P=0.018).The scores of the treatment group and the control group were compared on the 3rd day after surgery.The difference was not statistically significant(P3=0.200).The scores of anal marginal edema in the treatment group on the 7th and 14th day after surgery were lower than those of the control group during the same period,and the difference was statistically significant(P7=0.040,P14=0.000);Comparing within the group,the scores of anal marginal edema in the two groups decreased with the passage of treatment time,and the difference was statistically significant(F=44.815,P=0.000).3.Comparison of wound hemorrhage:There is a statistically significant difference in wound hemorrhage scores between the two groups(F=15.857,P=0.000).Among them,the wound bleeding scores of the treatment group were low on the 3rd,7th,and 14th day after operation.In the control group during the same period,the difference was statistically significant(P3,7=0.000,P14=0.045);comparison within the group,the wound bleeding scores of the two groups decreased with the passage of treatment time,and the difference was statistically significant(F=80.548,P=0.000).4.Comparison of wound exudation:the comparison of wound exudation scores between the two groups was statistically significant(F=7.76,P=0.007).Among them,wound exudation in the treatment group on the3rd,7th,and 14th day after surgery The scores were lower than those of the control group during the same period,and the difference was statistically significant(P3=0.039,P7=0.000,P14=0.045);comparisons within the group showed that the wound exudation scores of the two groups increased with treatment time However,the difference was statistically significant(F=71.503,P=0.000);there was no interaction between treatment time and group(F=1.948,P=0.129).5.Comparison of granulation tissue growth:The comparison of wound granulation tissue growth scores between the two groups was statistically significant(F=12.298,P=0.001).On the 3rd day after surgery,the treatment group and the control group had granulation tissue growth.The score comparison was not statistically significant(P3=0.130).On the 7th and 14th day after surgery,the granulation tissue growth scores of the treatment group were lower than those of the control group during the same period,and the difference was statistically significant(P7=0.000,P14=0.049);In comparison within the group,the granulation tissue growth scores of the two groups decreased with the passage of treatment time,and the difference was statistically significant(F=100.971,P=0.000).6.Comparison of wound size:There was no statistically significant difference in wound size between the two groups(F=1.001,P=0.320),and the comparison of wound size between the treatment group and the control group on the 3rd day after surgery was not statistically significant(P3=0.607),there was a statistically significant difference in wound size between the treatment group and the control group on the 7th and 14th day after operation(P7=0.033,P14=0.002);comparison within the group,the two groups The wound area decreased with the treatment time,and the difference was statistically significant(F=880.869,P=0.000).7.Comparison of average wound healing time:The average wound healing time of the treatment group was 16.36±1.85d,and the average wound healing time of the control group was 18.10±2.13d.After comparison between the two groups,the difference was statistically significant(P<0.05).8.Overall curative effect comparison:The two groups were compared on the 14th day after operation.The total effective rate of the treatment group was 94.73%,and the total effective rate of the control group was87.17%.Comparison of clinical efficacy between the two groups after treatment,the difference was statistically significant(P<0.05).Conclusion:The application of hemorrhoid anti-inflammatory ointment gauze strips to dressing change after internal sphincter release in anal fissure can effectively reduce postoperative anal pain,anal edema,wound hemorrhage and exudation,and promote the growth of wound granulation tissue.,Shorten the wound healing time,improve the clinical cure rate,prove its safety and effectiveness,less side effects and other advantages,it is worthy of clinical promotion and use.
